The purpose of this visit was to conduct a routine inspection. Please provide a CFM photo ID to the Health Department with in 7 days. ORS Inc. information is below:

  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action. DISCUSSED WITH THE MANAGER. INFORMATION PROVIDED. PROVIDE A COPY OF CFM PHOTO ID WITHIN 7 DAYS.
  • Light Bulbs / Locations where Shielding is Required (corrected on site)
    Observation: Tube bulb(s) at the cilling (front of the HOBART Freezer) were not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. DISCUSSED WITH THE MANGER. THE MANAGER WILL COVER THE LIGHTS.

NOTE:
- Monitor sanitizer (Chlorine) concentration once a day.
- When you run the first load of dish, make sure the temperature reaches 120F (monitor temperature gauge). Call the dishwasher service contractor and fix the existing issue and send the receipt to HD.

  • Warewash Machines/Temp Meas Devices/Required for W,R,S (corrected on site)
    Observation: The temperature measuring device for the dishwasher machine is not operating properly. Dish washer machine needed about 4 run before the temperature gauge read 120F.
    Correction: A warewashing machine shall be equipped with a temperature measuring device that indicates the temperature of the water in each wash and rinse tank and as the water enters the hot water sanitizing final rinse manifold or in the chemical sanitizing solution tank. Advised Person-in-Charge to monitor temperature.
  • Sanitizing Solution Test Kit Accurate & Used (corrected on site)
    Observation: Observed that Person-in-Charge has been using wrong type of test kit on the sanitizer bucket and 3-vat sink which led to misreading of quaternary ammonium sanitizer solution.
    Correction: The employees must be monitoring the concentration of the sanitizing solution using a correct test kit or other device. Quaternary ammonium solution test kit is used to test sanitizer solution at the sanitizer buckets and 3-vat sink,

The purpose of this visit was to conduct a routine inspection. Water Heater information: State Sandblaster: SBS10076NE. PIC to obtain certified food manager's card, and FAX/email copy to health department within 10 days ( FAX:703-278-8157 Rick.Kurtz@FairfaxCounty.gov . PIC to have all employess trained in Employee Health Policy details, and sign an employee illness reporting agreement also within 10 days.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy." Note: PIC had and understood policy, but no signed employee reporting agreements.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Fairfax City Code/Certified Food Manager/Certification Process
    Observation: The Person in Charge (PIC) has proof of successful completion of an approved Certified Food Manager (CFM) exam but does not possess the Northern Virginia Certified Food Manager certificate (photo card) issued by ORS Interactive, Inc.
    Correction: Obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days to avoid further enforcement action. A copy of the photo identification card can be faxed to my attention at (703) 278-8157 or emailed to Rick.Kurtz@FairfaxCounty.gov
  • Physical Facilities Good Repair
    Observation: Observed that two ceiling tiles near Hobart freezer are missing.
    Correction: Replace the missing ceiling tiles. Poor repair and maintenance compromises the functionality of the physical facilities.
