Eliminate pest from facility. Eliminate harborage conditions such as grease between the equipment, old food debris in gaskets of low boy refrigerator.
Operator provided the pest control service report from 6/16/15. Continue the pest control service.

During the inspection five live roaches were observed in the facility. One on floor near hand washing sink, one on corner of shelf above low boy refrigerator, one under microwave, two baby roaches on front wooden service counter.
Operator provided the pest control report from 6/10/15 which stated the areas inspected and serviced. Pest control services must be continued till roaches in the facility eliminated. Facility must be cleaned thoroughly. Follow-up inspection scheduled for 6/17/15.

A complaint was received stating that roaches were crawling on walls in Pattaya Thai.
During the inspection six live roaches were observed crawling on inner corners of shelves above two door low boy refrigerators and inner corners of food prep table next to dishwasher. Several dead roaches observed on floor under and near dishwasher and stoves observed greasy with old food debris. Dead roaches remains observed in cracked cover of reach in freezer.
Complaint found. Operator instructed to work with pest control company to eliminate roaches from the facility. Clean the facility thoroughly.
Follow up scheduled.

Complain was received stating that a roach was crawling on the complainant's table at said establishment. The manager was unaware of the complain. During the inspection four dead roaches were found on floor in kitchen. Three live roaches were found in corner under the food prep table next to dishwasher. Grease, old food debris and water observed on floor in kitchen, condition conducive to pest harborage. As per the manager the last pest control was done on 6/24/2014.
Complain was found.
Operator voluntarily scheduled a pest control service for 7/17/14. Operator will be providing the receipt and report from pest control. Operator shall voluntarily clean and sanitize the kitchen.
Follow up inspection scheduled.
