  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Utensils in dipper well without water running water.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently. MANAGER TURNED THE DIPPER WELL FAUCET ON TO FLUSH PARTICLES OFF THE UTENSILS.
  • Food Temperature Measuring Device Required & Readily Accessible
    Observation: The certified food manager could not provide a working food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Equipment / Non Food / Design / Easy Clean
    Observation: Milk and soda crates were found used as racks for the storage of supplies in the walk-in and dry storage areas including the soda syrup closet.
    Correction: Milk crates, soda crates, and/or bread racks are not designed to store or elevate food, equipment, utensils, clean linens, and single-service items. Provide only ANSI-certified (such as NSF-listed) shelving units, storage units, or dunnage racks. Replace the crates accordingly.
  • Warewash Equipment / Cleaning Frequency
    Observation: The exterior surfaces of the were washing machine have lime accumulations and the top of the machine has debris build-up.
    Correction: Clean the ware washing machine more often to prevent debris build-up. Delime the exterior surfaces of the machine to reduce lime build-up.
  • Equip/Utensils/Linens/Single-Service/Original Package
    Observation: Plastic beverage cups were observed stored unprotected at the serving counter.
    Correction: Single-service items shall be stored in a clean, dry location that is not exposed to splash, dust, or other contamination and at least 6 inches off the floor on approved shelving units or dunnage racks. In addition, the single-service items shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
  • Lighting Intensity/ Food Prep Area / 50 foot candles
    Observation: Observed that inadequate lighting was provided in the food preparation areas. Some of the ceiling light fixture bulbs were observed burned out.
    Correction: Increase the lighting to provide a minimum of 50 foot candles in all food preparation areas. Replace the burned out bulbs to provide proper lighting.
  • Mechanical Ventilation Required
    Observation: Ventilation is not sufficient to keep rooms free of excessive obnoxious odors. The vent system in the women's restroom is not operating properly.
    Correction: Provide mechanical ventilation at a sufficient capacity to efficiently remove obnoxious odors. Fix the ventilation in the women's restroom.

08/28/2013Risk Factor
The purpose of this visit is to conduct a complaint inspection. The manager stated that she was not aware of the complaint. She stated that the customer self service area is cleaned on a daily basis and described the process of how the nozzles are cleaned and sanitized. During the inspection the manager observed black debris on the ice dispenser and immediately cleaned the area. She stated that she will ensure that the customer self service area is thoroughly cleaned every shift. Complaint is confirmed.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (corrected on site)
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required. OBSERVED BLACK DEBRIS ON ICE DISPENSER.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold. MANAGER CLEANED ICE DISPENSER AND NOZZLES.

The purpose of this visit is to conduct a routine inspection in conjunction with a complaint investigation. Upon arriving at the establishment the manager was not aware of a complaint but stated that a few weeks ago there was a disgruntle customer and believes the complaint may be related. It was observed the inspection that the facility was free of excessive debris around the sandwich prep areas. The manager stated that cleaning is done periodically throughout the day. During the inspection frequent hand washing was observed as well as changing gloves and all employees were wearing a hat or hair net as a hair restraint. There were no violations observed.

The purpose of today's visit was to conduct a risk factor assessment.
No violations were observed today. CFM was able to describe employee health policy, which is documented with an internal computer program. All temperatures observed today were all within range, the facility was observed clean and well-organized despite being very busy. Multiple employees were observed washing hands correctly and wearing gloves as necessary.
