Observation: The chemical sanitizer was too strong. Observed above 200ppm, employee added more water to basin to bring concentration to 200ppm.
Correction: Obtain a quaternary ammonium chemical test kit to ensure the sanitizing solution is maintained at the proper solution.

Observation: Observed accumulations of grime and debris on the following food contact surfaces: interior of ice machine.
Correction: Clean and sanitize these surfaces for food contact. PIC unplugged machine to allow it to drain and be cleaned.

Observation: Dead insects were found on the premises. Observed under prep table, staff removed.
Correction: Remove dead or trapped birds, insects, rodents, and other pests from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

New Food Code changes given and new Form 1-B issued. Kitchen manager and custodian will continue to address any insect issues and make sure that the hole gets covered.

Observation: Methods are not being used to control pests. Hole observed in floor near dishwasher with insects in it. Spoke with custodian and he applied a treatment in hole. Kitchen manager had put work order in last year to get hole covered but the work was never completed, so she called in another order.
Correction: When pests are found use methods to such as trapping devices or other means of pest control. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

Spoke with CFM about putting in a work order on the Traulsen fridge by the hot box. The ambient temp was high but a lot of traffic in and out during operational hours. Temp logs reflect proper function but possibly lowering the setting will help keep it cooler during peak hours. Kitchen neat and clean.

Observation: A food service employee was working in the food service establishment without having a valid food service card. Attended class in March but does not have copy of card in facility or on person.
Correction: A food service employee shall have a valid food service card prior to working in the food service establishment. The person in charge shall not employ anyone to work in the food service establishment until they posses a valid food service card. A food service employee shall have a valid food service card while working in a food establishment.
- Critical: Food - Potentially Hazardous Food: Cold Holding* (corrected on site)
Observation: Cheese sanidwich cold holding at improper temperatures. Made the previous day, PIC discarded.
Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
- Temperature Measuring Devices
Observation: There was no temperature measuring device observed located in the Good Humor ice cream freezer.
Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
- Equipment and Utensils, Air-Drying Required
Observation: Lunch trays were found stacked wet after cleaning and chemical sanitization.
Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
