LEX#1431327 A complaint was received that stated that the complainant's son and his girlfriend became ill after consuming a meal from this facility. PIC was not aware of the complaint. When asked PIC stated that no employees have called out sick or been sent home sick within the past two weeks. The Health Department employee flowchart is posted in the facility and the PIC was able to show it to EHS without delay. PIC further stated that new employees are trained on the flowchart and are provided with a copy at hire. EHS had requested sales numbers for foods prepared in the restaurant for Thursday, Friday and Saturday. Some ceiling tiles are not properly seated (store front side and long side of hood) (correct prior to reopen) Ice machine - have serviced/cleaned prior to resuming use of ice. Observed paint cans improperly stored - please check for other supplied left by contractor and relocate as needed. Ladder stored as backdoor - please clean within 90 days. Please call or email if you have any questions -> facility plans to reopen on Saturday.

Site visit made due to report of an incident last night. The owners were on site. An air handling unit was blown over and allowed rainwater into the building and gas and water lines in the ceiling were broken. The gas and electric service is currently turned off in the building. There is water damage along the right wall of the facility and in the ceiling. A professional cleaning/remediation firm has been hired. Ceiling tiles/light fixtures/vent hood/walls and cove base need to be inspected/cleaned as needed. Any food on the floor or items that got wet shall be discarded. All foods in coolers/freezers to be discarded (PHFs). Please stay in tough and contact EHS for an inspection prior to reopening (need to check coolers). Correct the remaining item today, maintain the corrected items as corrected. Discussed 240 hair restraints with facility owner. Please increase the cleaning frequency of interior ceiling of ice machine. New quarry tile floor in September 2014 - great!

Maintain the corrected items as corrected. Reviewed cooling and cooling methods with PIC. Reminder: when removing vacuum sealed fish from the freezer cut the corner to break the seal. Reminder: when testing quaternary sanitizer solution should be about 75°F. Overall clean facility.
