    Observation: The chest freezer was observed in a state of disrepair and damaged.
    Correction: Repair the chest freezer to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Correction: Cover all waste containers when not in continuous use.

    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents - screening on windows.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The following physical structures were noted in need of cleaning: floor along walls and under equipment and wall at the handwashing sink.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner. The painted block walls at the handwashing sink are difficult to clean.

    Observation: The following equipment was observed in a state of disrepair and/or damaged: chest freezer, warmer door, gaskets to refrigeration units, walk-in freezer, and a stainless table / duct (duct taped because of sharp edges/points).
    Correction: Repair the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The thermometer on the METRO CS warmer is not in good repair and/ or not accurate in the range of use.
    Correction: Repair, replace, and/or maintain the calibration of gauges for accuracy and function in the intended range of use. Place an oven thermometer in the unit until repaired.
  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections under the handwashing sink are leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Physical Facilities in Good Repair
    Observation: The walk-in cooler floor is not maintained in good repair - corroding.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils or after engaging in any activity which may have contaminated his/her hands.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

  • Sanitizing Solutions, Testing Devices
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a @SANITIZING AGENT@ test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The door gasket to the Raetone Supreme (gold) is in poor repair. Inspect freezer units also. The walk-in freezer has moisture dripping from the ceiling inside of the unit.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications. The walk-in freezer must be operating properly. The drain from these units may be causing the black substance on the ceiling in the storage room.

    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents - back door.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2.     Observation: Less than 10 foot candles of light was noted in the walk-in - 5 foot candles.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.
  • Physical Facilities in Good Repair
    Observation: The following physical structures are not maintained in good repair: ceiling tiles removed, walls at the dishwasher spray area and handsinks, ceiling in the storage room appears to have a black substance (possibly from moisture), and floor at the ice maker..
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
