Management states that the space under the mopsink has been partially cleaned out. Inspector could not view this space as the access port had been replaced and a maintenance employee from P & G was working on the mopsink dispensing unit. Management also states that the plumbing under the mopsink has not been repaired. The intense, rancid odor in the kitchen near this mopsink has not been alleviated.
Roof leak has still not been repaired. A copy of the communication between store manager and a supervisor was provided to inspector, documenting that supervisor was informed of leaking roof 08-June-2015.

  • Plumbing System Maintained in Good Repair (repeated violation)
    Observation: Plumbing connections under the mop sink are leaking and / or backing up into the space under the sink, which is a walled-over enclosure with a cleanout port. Odor was intense in the kitchen, and thick piled up sludge stood 8"deep (grease waste??) around the sink drain. Pooled wastewater stood on top of the sludge and may begin to move out into the restaurant with continued use.;Plumbing systems and components shall be maintained in good repair. Open up and clean out space under the mopsink. Eliminate all residues and sanitize the area. Repair damaged drains.

    Observation: The hot water tap at the mopsink will not shut off, causing continuous pressure to be applied to the bell breaker backflow device.
    Correction: Repair hot water valve. Ensure pressure is relieved from system after each use by turning off water and draining all valves.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Roof is not maintained in good repair. A roof leak that was noted in May 2014 has still not been repaired. Roof continues to leak after heavy rainfall events, introducing contamination to the kitchen and dining areas.
    Correction: Maintain physical facilities in good repair. Roof leaks introduce contamination to the facility and encourage the growth of microbes. Ceiling tiles in area are damaged.
09/01/2015Follow-up
Roof leak has still not been addressed. It is now more than a year since the problem was first noted. An additional problem, a drain failure, has been allowed to continue unabated while intense odor and heavy sludge accumulations build up under the mopsink.
  • Utensils - In-Use - Between-Use Storage
    Observation: Ice scoop is stored with the handle buried in the ice. Ice machine is damaged and overflowing ice to the floor.
    Correction: Store ice scoops so that the handle does not touch the ice. Add a storage bin outside of the ice machine to provide protected storage for the scoop.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Inner door and the floor of the walk-in cooler were observed to have accumulations of soil and organic residues.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Critical: Equipment - Food Contact Surfaces and Utensils
    Observation: Equipment food contact surfaces/utensils are being cleaned of grease residues at the mopsink. This mopsink is in active use as a mopsink. Scrapers that are used on the trays are being stored in soiled condition, jammed under boxes on the shelf over the mopsink. Accumulated congealed fats stain the boxes and line the shelf edge.
    Correction: Clean food contact surfaces of equipment and utensils between and after uses to prevent cross contamination.

Note that since previous inspection, operator has relocated the coffee machine to a protected location, freeing up the sideboards of the 3CS for proper handling of dishes.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils* (corrected on site)
    Observation: The food contact surfaces of the produce slicer had dried residues from the last use several days ago. Chunks of drying onion were adhered to the blades.
    Correction: Clean and sanitize these surfaces for food contact.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Roof is not maintained in good repair. Roof leaks during wet weather, with rainwater dripping to the kitchen floor in the narrow passageway between front kitchen and back (heavy traffic area). Contamination of floors and risk of contamination of food being carried through the area. A ceiling tile in the kitchen, and another in the dining area have visible water damage and should be replaced.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Repair roof leak immediately. Replace damaged tiles in kitchen and dining area.
05/12/2015Routine
Please note that dishware is to be washed at the 3-compartment sink (3CS). Storage of soiled dishes while awaiting washing may be in a compartment of the 3CS, on the drainboard of the 3CS, or on a cart or table. Dishes may be rinsed of coarse soil in the 3CS, then processed as normal through the wash/rinse/sanitize procedure. If pre-rinsing of coarse soil causes debris in the sink, be sure to clean the sink, rinse it, and sanitize it before using the chamber for wash/rinse/sanitize. As there are at present no other sinks available for warewashing, any alternative location for storage of soiled dishes, or pre-rinsing of soiled dishes, would constitute facility changes and hence must be approved by your VDH inspector.
Note for future inspections: this problem may have arisen as a result of conflicting instructions between SCR (a LJS corporate-ordered inspector) and requirements from VDH. It is reported that SCR has refused to allow staff to pre-rinse coarse soil from the dishes at the 3CS or to store soiled dishes at the 3CS. There are no other dishware sinks, and no available surface for storage of soiled dishes. At this time, the 3CS is the only option.

  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation*
    Observation: Coffee machine is set up and hard-plumbed on 3-compartment sink drainboard, with no protection from spray/splash/drainage incurred during warewashing. Presence of coffee machine is blocking proper use of 3CS. Food prep equipment such as coffee machines must be installed in food-prep areas, away from areas that will introduce contamination and adulteration.
    Correction: Relocate coffee machine to proper food-prep area.
  • Equipment - Dispensing Equipment, Protection of Equipment and Food
    Observation: The soda fountain nozzles are not properly cleaned and have no sanitizing process. Operators clean soda nozzles with carbonated water only and re-install.
    Correction: Ensure that soda nozzles are properly cleaned and sanitized clean with adequate frequency to prevent accumulation of organics and growth of microbes.
  • Prohibitions
    Observation: Soiled dishes were observed stored in the mop sink. Management states that a company inspector instructed them that soiled dishware must be placed in the mop sink or in the floor. Mop sink is in use as a mop sink---mop buckets are emptied here. A sign on the 3-compartment sink (3CS) instructs employees that soiled dishes cannot be placed in the 3CS, but shall be placed in the floor. 3CS drainboard is blocked by the presence of an in-use, hard-plumbed coffee machine, preventing employees from being able to use drainboard for stacking dishes.
    Correction: Discontinue storage of soiled dishes in mop sink or in floor. Protect food-contact surfaces from unnecessary contamination such as will be experienced when contacting heavily contaminated surfaces of a mop-sink. Mop water will contain gross filth and high microbial loads and should never be sharing work surfaces with food contact equipment.
  • Physical Facilities in Good Repair
    Observation: Roof is not maintained in good repair. Two actively dripping roof leaks were witnessed at inspection, one in the kitchen dripping straight to unobstructed floor surface (no food prep or storage at or near location), and the other in the dining area where booster seats and a booth had standing water. At inspection, it had been raining for several hours.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Roof leaks pose a threat of introducing contaminants and adulterants to the kitchen and dining areas.
