Observation: A food service employee was working in the food service establishment without having a valid food service card.
Correction: A food service employee shall have a valid food service card prior to working in the food service establishment. The person in charge shall not employ anyone to work in the food service establishment until they posses a valid food service card. A food service employee shall have a valid food service card while working in a food establishment.

Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food, including the date of onset of jaundice or of an illness due to Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus.
Correction: Have the license holder require employees or applicants report to the person in charge any information about their health and activities if they are diagnosed with or had a past illness from Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virusor Norovirus.Employee Health form to be filled out and signed by each employee.Form issued to owner at pre-opening with Supervisior of Health.

Observation: Unlabeled food containers.(Squeeze Bottles)
Correction: Label working containers with the common name of its contents. Food containers shall be labeled in a language that is understood by the regulatory authority and by the employee. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
- Food Storage - Clean and Dry Location
Observation: Food stored on the floor. Or Food stored less than 6"" above the floor. (Walkin cooler).Onions and Sweet Potato Bags stored on floor in walkin cooler.
Correction: Elevate food storage onto approved shelving with minimum 6"" legs or casters.

Observation: The chemical sanitizer was too weak.(3 compartment sink- 0 ppm QAC reading)
Correction: Notify chemical sanitizer company tech rep, to ensure the sanitizing solution is maintained at the proper solution, and equipment is operating at manufactures specifications.Wash
- Equipment and Utensils, Air-Drying Required (corrected on site)
Observation: Serive Bowels,strainers,and pans were found stacked wet after cleaning and chemical sanitization.
Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
- Tableware - Preset Tableware
Observation: Present tableware was observed unprotected from contamination.(Not covered or indivdually protected).
Correction: Discontinue practice of unprotected tableware/utensils from contamination by maintaining items wrapped, covered.
- Handwashing Cleanser; Availability (corrected on site)
Observation: Soap was not provided at the hand washing lavatory in the two up front hansinks.
Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
- Physical Facilities in Good Repair
Observation: Cover plates in ladies restroom and at dishwasher wall area are not maintained in good repair. Also wall at walkin refrigerator,prior to access to walkin cooler is in disrepair.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.(Cover plates have gaps,hole area around plate/exposed wall access).Correct/fix wall at walkin cooler.
