The purpose of this inspection was to conduct a routine risk evaluation. Chlorine sanitizer in 3 vat sink: 75 ppm. NOTE: PLEASE REMEMBER TO PROVIDE COPY OF CURRENT ESTABLISHMENT PERMIT TO HEALTH DEPARTMENT WITHIN 10 DAYS.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Permit to Operate Required, Must be Posted, Must be Valid (corrected on site)
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue. NOTE: PIC agreed to locate/obtain current permit and post in conspicuous location. PIC will provide copy of the permit to the Health Department within 10 days.

The purpose of this visit was to conduct a risk factor assessment. Chlorine sanitizer concentration in 3 vat sink = 50 ppm. Good retail practice discussed during inspection: refinish/replace cabinet bottom under sink to provide smooth, easily cleanable surface.

The purpose of this visit was to conduct a routine risk inspection. Water heater information: State Sandblaster S8S6565NE. Chlorine sanitizer in 3 vat sink: 0ppm/50ppm, initial/final. PIC provided invoice for latest pest control service on 3/21/13, and advised pest control service is provided monthly. Hood last serviced 8/2012, and is serviced every 6 mo per documentation on hood. PIC to send documentation to Health Department by 4/18/13 confirming that handwashing sink and 3 vat sink are provided with the required hot water temperatures. NOTE: PLEASE REMEMBER TO PROVIDE DOCUMENTATION TO HEALTH DEPARTMENT CONFIRMING REQUIRED HOT WATER TEMPERATURES AT HANDWASHING AND 3 VAT SINKS PRIOR TO 4/18/13, AS REQUIRED ABOVE.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3 vat sink with a concentration of 0 ppm total chlorine. NOTE: PIC raised sanitizing solution concentration to 50 ppm.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.
  • Manual Warewashing Wash Solution Temperature 110°F or per Manufacturer label
    Observation: The temperature of the in-use wash water solution was observed at less than 110°F. Maximum temperature at three vat sink measured to be 102 degrees F.
    Correction: The temperature of the wash solution in manual warewashing equipment shall be maintained at not less than 110°F or the temperature specified on the cleaning agent manufacturer's label instructions.
  • Handwashing Sink / Mix Valve or Combination Faucet , Water at 100°F
    Observation: Water from the handwashing sink at handwashing sink was measured at a temperature less than 100°F. Handwashing sink temperature measured to be 93 degrees F. NOTE: PIC will have temperature corrected and provide documentation to Health Department.
    Correction: A handwashing sink shall be equipped to provide water at a temperature of at least 100°F through a mixing valve or combination faucet.
