The purpose of this visit is to conduct a routine inspection. Remove all debris surrounding the outside dumpster area.
- Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: Observed raw shelled eggs stored over milk containers, yogurt and hummus in the 2dr True Refrigerator.
Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. EMPLOYEE MOVED THE RAW SHELLED EGGS TO THE BOTTOM SHELF AND PLACED OTHER ITEMS ABOVE.
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.
- Refuse / Area / Clean
Observation: Trash and litter were observed adjacent to the refuse container outside the facility.
Correction: A storage area and enclosure for refuse, recyclables, and returnables shall be maintained free of unnecessary items and kept clean.

The purpose of today's visit was to conduct a routine inspection. No critical violations observed today. Please obtain a quaternary ammonium test kit. Thermolabel activated on the Jackson Avenger heat sanitizing dishmachine. Please note that today is the third time a row no quaternary ammonium test kit was available during the inspection. Please consider ordering extra test kits. Also please note once the quaternary ammonium solution is prepared in sanitize basin the water temperature needs to be between 65 F-75F (room temperature) before testing or there may be a false reading of too low or too high. EHS provided facility handout on date marking in English and Spanish and handout on how to calibrate a thermometer. Once the test kit is obtained, please email or fax a copy of the receipt or invoice to EHS no later than ten calendar days, thanks.
- Temp Meas Devices/Food & Mech. Warewasher/ Scaled in Range of Use
Observation: The food temperature probe (is improperly scaled, not in the intended range of use for measuring hot/cold food temperatures).------Obseved facility has food thermometer ranging between 140F - 190F in increments of 10F.
Correction: Food temperature measuring devices shall have a numerical scale, printed record, or digital readout in increments no greater than 1°C or 2°F in the intended range of use.
- Sanitizer Test Kit Required (repeated violation)
Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
Correction: Obtain a QUATERNARY AMMONIA test kit. Note: facility is waiting for end of month order of test kits to arrive.

The purpose of this visit was to conduct a risk factor assessment. Dishmachine turned thermolabel black. The following good retail practice were discussed during the inspection: obtain quaternary ammonia test strips for 3 vat sink sanitizer.

Observation: Raw animal food stored over ready-to-eat food in the refrigeration unit. Raw hamburger stored over RTE foods in True 2 dr upright cooler.
Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc. NOTE: PIC corrected storage order.

Employee health forms provided to Director, however, based upon the program standards, the facility has an adequate employee health policy as part of the regular training for staff.
