The purpose of today's visit was to conduct a risk factor assessment. Violations observed were corrected during the time of the inspection. In general, good active managerial control measures are in place. Employee training meeting is scheduled for this evening.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: CHEESE SLICES and COLE SLAW
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
    ITEMS WERE LABELED WITH DISCARD DATE.
  • Critical: Storage of Toxics, Not Above Food, Linens, Equip., Single-Svc.
    Observation: (CORRECTED DURUNG INSPECTION) Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items OBSERVED SPRAY BOTTLES OF CLEANERS HANGING FROM SIDE PANEL OF THE HANDSINKS IN THE FRONT SERVICE AND DISH WASHING AREAS.
    Correction: All poisonous and toxic materials shall be stored below food, equipment, utensils, linens, and single service items to prevent contamination.
    RE=LOCATE THE SPRAY BOTTLES TO THE BACK STORAGE SHELF WITH THE OTHER CLEANERS.
12/09/2015Risk Factor
Conducted a follow-up inspection to the routine inspection performed on June 1, 2015. Found that the Certified Food Manager (CFM) has provided training on food borne illness and prevention for all of the food employees. A copy of the signed Employee Illness Reporting Agreement form for the food employees is now on file at this establishment. Also, the manager has obtained the Certified Food Manager license (photo ID). The establishment is now in compliance for violations 2-210.11 (A) (1)-(5) and 43.1-3-4 (a) -(h) cited during the previous routine inspection.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Light bulb(s) in the CEILING PANELS THROUGH THE FRONT AND BACK KITCHEN AREAS ARE NOT COVERED BY PROTECTIVE SHIELDING.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.
    PROVIDE PROTECTIVE SHIELDING FOR ALL OF THE CEILING LIGHT PANELS.

The purpose of today's visit was to conduct a routine inspection. Manager has passed Serv Safe Certified Food Manager Exam but does not have the Certified Food Manager License. The Manager will obtain the Certified Food Manager Photo ID within ten (10) days. Also, food employees have not been provided training on food borne illness and prevention. Training will be provided this week and documentation of the training will then be provided to the Health Department.
  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy."
    FOUND THAT THE FOOD EMPLOYEES HAVE NOT RECEIVED TRAINING ON FOOD BORNE ILLNESS AND IT'S PREVENTION. EMPLOYEE HEALTH POLICY HANDOUTS (RED FOLDER) WAS GIVEN TO THE MANAGER. REVIEWED THE HANDOUTS WITH THE MANAGER.

    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: OBSERVED ICE SCOOP STORED ON TOP OF THE ICE MACHINE LOCATED IN THE BACK KITCHEN AREA.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.
    ICE SCOOP NOW STORED ON CLEAN TRAY.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    FOUND THAT THE MANAGER HAS PASSED SERV SAFE FOOD MANAGER EXAM. HOWEVER, THE MANAGER DOES NOT HAVE THE CERTIFIED FOOD MANAGER LICENSE.

    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.
    MANAGER AGREED TO OBTAIN THE CERTIFIED FOOD MANAGER PHOTO ID WITHIN THE NEXT TEN (10) DAYS. IT WAS ALSO AGREED THAT A COPY OF THE PHOTO ID WILL BE FAXED TO THE HEALTH DEPARTMENT ONCE IT IS OBTAINED.

PREOPERATIONAL FINAL INSPECTION
1. Self-serve soda dispensing machine has short levers that are in violation of Section 4-204.13 (D) of Chapter 43.1: Food and Food Handling Code. Install "Push Buttons."
2. Final mechanical inspection has not been obtained.
3. Three light fixture shields over the kitchen area have not been installed.
4. A splash guard has not been installed between the food preparation sink and the clean dish drainboard of the three-compartment sink.
5. The right and left sides of the walk-in refrigerator/freezer have not been sealed to walls.
6. The hood has not been caulked to wall.
7. The self-serve B.B.Q. sauce station has no food protectors/sneeze guards. Install sneeze guards of pump type B.B.Q. sauce dispensing unit.
8. The upright hot holding unit has a broken door handle.
*Owner/Contractor shall correct the above listed items and schedule for a final follow up Health Department inspection.
