Observation: Smoked salmon is on the menu. At the time of inspection, the manager could not provide information that the smoked salmon is farm raised. It is believed that this information is on the commercial packaging of the smoked salmon.
Correction: Take a picture of the package indicating origin of the smoked salmon and fax or email the information to the Health Department within 10 days.
- Critical: Common Name on Working Containers of Toxics (Cleaners, Sanitizers) (corrected on site)
Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name.
Correction: Label all working containers of poisonous and toxic materials, including cleaners and sanitizers, with a common name to prevent the dangerous misuse of these materials. Spray bottle of cleaner was properly labeled.

This was a routine inspection. Datemarking was being used to identify the "cut" date of deli meats that were cut and place in the freezer for storage. It was recommended that the date the meat is taken from the freezer be marked on the packages as that is when the refrigerated storage timer of 7 days starts. Do not keep sliced meats refrigerated for more than a total of seven days, whether frozen for a period of time or not. It was also recommended to turn the thermostat down on the 3DR prep unit. Ambient air temperature was 40F - food was measured at 42F inside the box.
- Hair Restraint Effectiveness
Observation: Employees observed working in the food service area without proper hair restraints.
Correction: Provide hats or other hair restraints for employees involved in food preparation.
- Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
Observation: Sliced ham and turkey were datemarked incorrectly. The date was not indicated as to when the product would be discarded but only the date the product was cut.
Correction: When the deli meat is sliced and frozen and then taken from the freezer to be used, put the date the meat was taken from the freezer and thawed as the date to count from for the time to discard. The date should be six days from the date the meat was taken from the freezer. Deli meat was re-marked with correct dates.

Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris:------dry storage shelving in back.
Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.

Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at men's restroom handwashing sinks used by food employees.

Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: provolone cheese, american cheese, sliced and individually packaged from bulk.
Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
- Outer Openings, Protected / Screen Requirements
Observation: Openings to the exterior of the building are present along the back door screen.
Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents.
