    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

    Observation: Cooked Meat noted not being adequately cooled to prevent the growth of harmful bacteria.
    Correction: Cool potentially hazardous foods from 135°F to 70°F within 2 hours and within a total of 6 hours from 135°F to 41°F. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. Corrected during inspection.

    Observation: Food containers for cooling food are arranged so as not allow for maximum heat transfer.
    Correction: Arrange the food containers in the refrigeration equipment to provide maximum heat transfer through the container walls. Loosely cover or uncover the food if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food.

    Correction: Prevent BIOLOGICAL CROSS-CONTAMINATION by storing food in packages, covered containers, or wrappings.

    Observation: In-use utensils improperly stored between use. Not using dispensing utensils with handles to scoop foods and leaving in the food containers.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands (2) The container and (3) Exposed food clean equipment, utensils, and linens and unwrapped single-service and single-use articles.
  • Critical: Reheating for Hot Holding* (corrected on site)
    Observation: The spaghetti meat ball sauce was not reheated to 165F within 2 hours to eliminate pathogenic bacteria. The sauce was placed in the hot holding area with temp only at 129 to 145F when tested at beginning of Inspection after being reheated for about 45 min.
    Correction: Was moved back to the stove and corrected to above 165F withing 45 minutes. Rapidly reheat food for hot holding to 165°F or above within 2 hours.
  • Food Contact Surfaces - Cleanability*
    Observation: The food contact surface of the dough storage containers have holes in them from usage, not smooth and contains cracks, chips, or pits that can not be easily cleaned.
    Correction: Replace the damaged containers to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose
    Observation: The handwash station at the Dishwashing area is being used as a dump station.
    Correction: The handwash facility identified above is to be used for washing hands only
  • Refuse - Covering Receptacles (repeated violation)
    Observation: Dumpster or outside refuse container was open or uncovered.
    Correction: Cover all waste containers when not in continuous use.
  • Refuse - Using Drain Plugs
    Observation: The refuse container used to store refuse/recyclables has no drain plug.
    Correction: Replace the drain plug to the refuse container.
  • Refuse - Maintaining Refuse Areas and Enclosures
    Observation: Trash and litter were observed adjacent to the refuse container outside the facility.
    Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.
  • Hand Drying Provision
    Observation: No disposable towels were provided at the hand washing lavatory in the upstairs location.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: Repair the ceiling in the Office / Storage area, observed part of ceiling surface had been removed.
