Observation: The ambient (air) temperature measuring device (thermometer) located in the frigidaire refrigerator is not accurate.
Correction: Calibrate thermometer per manufactuers instructions or replace with a thermometer that is easily readable and accurate in the intended range of use.

Self-closer is installed on door to break room just outside staff bathroom, but really should have self-closer attached to staff bathroom door itself to confine odors, bacteria and viruses.
Water quality standards had been in question, but see attached document showing all 2nd samplings are showing absent for bacteria.

Sunken spot on lower area of parking lot where dumpster has been stored. Engineer's evaluating. I printed off copy of repair sewage system permit for reference to where sewage drainfield lines are located in relationship to the parking lot. Also completed DSS Insp. report.

Observation: Employee observed working in the food service area without proper hair restraints.
Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food

If desire to have gloves that are easier to use, in handling food and other things, I suggest you try Nitrile or equivalent type gloves.
- Sanitizing Solutions, Testing Devices
Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
Correction: Obtain a Chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

Observation: The outdoor refuse container is stored on absorbent material. Gravel and grass.
Correction: All refuse containers are to be stored on smooth, nonabsorbent material that is sloped to drain. Suggest concrete pad.

Observation: The dumpster is missing drain plug near base.
Correction: Need install drain plug to keep vermin from gaining easy access.

Observation: Light bulbs in both refrigerators and standing freezer are not shielded, coated, or otherwise shatter-resistant.
Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.

Observation: Unlabeled food container - bulk flour and incorrectly labeled container of sugar (label states Teddy Grahams)
Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.

Observation: 2 boxes of plastic cups in side storeroom and one box of cups in back storeroom stored on floor.
Correction: Make sure to store boxes of single-service cups 6 inches off floor to avoid mop water contamination.

Observation: Toilet room doors are being kept open
Correction: Keep toilet room doors closed except when cleaning or during maintenance operations to prevent insect and rodent entrance and the associated potential for the spread of disease. Install self closer on STAFF BATHROOM door.

Correction: Provide a self-closer for the toilet room door. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
