The above observations and the following were discussed with the person in charge:
-Observed small divots in the blade of the slicer. Several areas are chipped and/or worn. Replace the slicer blade as soon as possible to prevent contamination and/or further chipping.
-Water pressure at the handsink in the back of the kitchen and on the left side of the serving line were both observed to be low. Repair as needed.

The above observations and the following were discussed with the person in charge:
-The door on the single door hot holding cabinet would not close unless pushed. Observed a gap along the side of the door. Foods maintaining 135'F and above during the inspection. Monitor the foods in the cooler and ensure the cabinet door is repaired or replaced.
-Replace the caulking behind the rinse sink by the dishmachine.
-A new ice machine has been provided for the kitchen. Ensure the backflow prevention device is replaced on the water line of the ice machine.
-The quaternary ammonium 146 sanitizer in the 3 compartment sink measured at 200 ppm.
The kitchen and all equipment was observed to be very clean and well maintained, great job!

    Observation: Milk observed to be cold holding at improper temperatures. Milk measured internally at 45'F in the right milk box. PIC stated this cooler was worked on last week and has been monitored. PIC turned the cooler down during the inspection.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

The above observations and the following were discussed with the person in charge:
-Thermometer calibration.
-A work order is in place to repair the dishmachine.
-Monitor the Hot Holding Cabinet to the left in the kitchen. When the door on the hot holding cabinet is opened on the serving line, the door in the kitchen opens and stays open until someone closes it. The gaskets were replaced and maintenance has been working to correct the issue. Recommend turning the temperature of the cabinet up during serving hours until the door can be repaired.
-The Quaternary Ammonium 146 Sanitizer in the 3 compartment sink measured at 150 ppm.
-The breakfast pizza was measured at 52'F one hour after being placed in the reach in cooler, excellent cooling procedures obseerved!

-Seal the handsink to the wall next to the warewashing area.
-Replace the blade on the can opener.
-Replace and/or seal the gaps in the screen on the back door of the kitchen.
-Slicer detailing.

Staff members are doing an excellent job with keeping the kitchen very clean, great job!

The above observations and the following were discussed with the person in charge:
-Employee health policy discussed, handout provided.
-The dishmachine was not in working order during the inspection. A work order is in place to repair the dishmachine.
-Ensure spray bottles are being relabeled with the common name when worn off due to washing.
-The quaternary ammonium 146 sanitizer in the sanitize buckets measured at 150 ppm and in the 3 compartment sink measured at 200 ppm.
Staff members are doing an excellent job with keeping the kitchen and all equipment extremely clean, thanks!

    Observation: The handsink along the front line and the prep sink in the back of the kitchen is not sealed to adjoining equipment or walls.
    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

    Observation: Vent filters in the hood system are not being maintained in a clean condition.
    Correction: Maintain hood system vent filters in a clean condition.
05/22/2013Routine
Discussed the following items with the Person in Charge:
1. PIC stated the milk box is left open during breakfast, closed and reopened and restocked just prior to lunch. The milk will be served or discarded prior to 4 hours from being removed from the walk in cooler. If air temperature on cooler does not drop, a work order will be placed. PIC also agreed to bring milk out to cooler in small batches until the cooler is capable of holding ambient air temperatures low enough to maintain milk at 41'F or below. Recommended ambient air temperatures are 38'F and below.
2. Observed one light bulb burned out inside the walk in freezer.
3. Facility was observed to be very clean and well maintained during today's inspection. Thank you!
4. Observed excellent hand washing and food handling during today's inspection.

    Observation: Milk (48'F it) cold holding at improper temperatures in the milk box on the serving line. PIC adjusted thermostat on milk box and will place a work order if the ambient air temperature in milk box does not drop.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
