    Observation: The person in charge was not available at the time of inspection. At the arrival of the inspection, there was no person in charge at the facility. The waitress contacted the person in charge and the person in charge arrived one hour later.
    Correction: The permit holder shall designate a person in charge who is authorized to monitor and manage all food establishment operations and who is authorized to take actions to ensure that the Regulations' objectives are fulfilled.

    Observation: The spicy spaghetti sauce (119'F), meat sauce (128'F), and marinara sauce (122'F) was not reheated to a sufficient temperature and time to eliminate pathogenic bacteria. Employee placed all sauces on the stove to reheat. All food items were properly reheated: Spicy spaghetti sauce (178'F), meat sauce (189'F), marinara sauce (201'F).
    Correction: Ensure potentially hazardous foods that are cooked, cooled, and reheated for hot holding are reheated so that all parts of the food reach 165°F or above for 15 seconds. Food shall be reheated within 2 hours.

    Observation: Garlic/oil mixture (47'F), alfredo sauce (47'F), sliced tomatoes (52'F), cooked chicken (52'F), cooked sausage (50'F), cooked penne pasta (46'F), cooked fettuccini noodles (47'F) were measured cold holding at improper temperatures in the pasta prep unit. PIC discarded all foods during the inspection. PIC lowered the unit temperature during the inspection.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: Raw and/or undercooked hamburger and steaks are provided on the menu, menu board, table tent or brochure without proper disclosure. Observed the food items that can served raw and/or undercooked are not properly asterisked on the menu. PIC stated she will asterisk the food items on the menus.
    Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods and identification of the animal-derived foods by asterisking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.

    Observation: Observed a food employee fail to properly sanitize large pieces of equipment in the 3-compartment sink. PIC instructed the employee on proper sanitizing and EHS provided training on sanitizer concentration and setting up the 3-compartment sink correctly. All equipment was properly sanitized during the inspection.
    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.

    Observation: No disposable towels were provided at the hand washing lavatory in the food prep area or the warewashing area. Paper towels were provided during the inspection.

Discussed with the Person in Charge:
1. Some updated food safety hand outs were left with the PIC to assist employees with preparing for upcoming food safety classes.
2. Several ceiling tiles were missing from above the dish machine due to a recent repair to the ventilation system. The repairs are not yet complete, and ceiling tiles will be replaced once the work is done.
3. Ensure prep cooler ambient air temperatures are holding between 36 and 38'F in order for foods to be maintained at 41'F and below.
4. Remove any unused or unwanted equipment from the facility.

    Observation: Observed the cook working in the food service area without a proper hair restraint.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles.     Correction: Separate raw foods during storage, preparation, holding, and display from raw RTE food including other raw food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Wiping cloths improperly stored between use. Observed a wet wiping cloth on the prep table to the left of the pizza prep cooler, and observed other wet wiping cloths in a bucket of soapy water with no measureable sanitizer.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Observed a bulk container of flour stored without a lid in the food preparation area.
    Correction: Protect food from contamination by storing the food in a clean and dry location with a cover over the container.

    Observation: Food containers for cooling food are arranged so as not allow for maximum heat transfer. Observed containers of pasta which were tightly covered during the cooling process. Significant amounts of condensation droplets were noted on the inside of the plastic wrap covering the containers.
    Correction: Arrange the food containers in the refrigeration equipment to provide maximum heat transfer through the container walls. Loosely cover or uncover the food if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food.

Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: The vinyl coving in the area of the soda dispenser is torn in one spot, and missing from the wall in another. The caulking along the crown moulding and the counter top in the area of the soda dispenser is beginning to peel away, leaving a gap between the counter and the crown moulding. There are several light bulbs noted to be burned out throughout the facility.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The floors throughout the facility along the floor and wall junctures are noted in need of a detailed cleaning. The wall under the prep sink is in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Discussed the following items with the Person in Charge:
1. Recommend attendance at food safety classes. A list of local classes and contact information was left with the person in charge.
2. Discussed option of using Time as A Public Health Control for the fresh garlic in oil. Guidance on this practice was given to the person in charge.
3. Ensure slicer, can opener blade and floor mixer are cleaned as soon as possible. Items were used just prior to inspection, as learned through discussion with the person in charge.
4. Recommend use of cooling wand to aid in faster cooling of sauce.

  • Hair Restraints - Effectiveness
    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles. This section does not apply to food employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) (repeated violation)
    Observation: Raw chicken (46'F it) cold holding at improper temperatures. Person in Charge instructed employee to return chicken to walk in cooler for faster cool-down. Also observed fresh garlic in oil left out at room temperature. Garlic in oil was re-located to the prep cooler.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Critical: Equipment and Utensils - Multiuse, Characteristics* (corrected on site)
    Observation: The food contact surface of the plastic grocery bag is not safe. Observed bread stored directly in a "to-go" style grocery bag in the prep cooler.
    Correction: Repair or replace the storage bags with equipment to permit easy cleaning and prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food.
  • Equipment - Non-Food Contact Surfaces and Utensils
    Observation: Interior surfaces of the ice machine were observed soiled with accumulations of grime and debris.
    Correction: Clean the surfaces of the ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
  • Equipment and Utensils, Air-Drying Required (corrected on site)
    Observation: Plastic drink cups and plastic food storage containers were found stacked while wet after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
  • Physical Facilities in Good Repair
    Observation: Several light bulbs throughout the facility are noted to be burned out.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
