    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: OBSERVED RAW SHELL EGGS BEING STORED OVER PASTRIES, PRODUCE , CREAM CHEESE AND BUTTER IN THE WALK IN REFRIGERATOR.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. DISCUSSED WITH CFM AND OWNER, EGGS TO BE MOVED TO LOWER SHELF.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: OBSERVED DOUGH MAKER, OVENS, PREP COUNTERS AND SHELVES USED FOR STORING FOOD WERE VISIBLY SOILED TO SIGHT AND TOUCH.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized. DISCUSSED WITH CFM TO CLEAN THE SURFACES ON A MORE REGULAR BASIS.
  • Certified Food Manager/Certificate Process (repeated violation)
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc. OBSERVED MANAGER HAS ONLY SERVESAFE CERTIFICATE. AS PER CFM CARD WAS LOST WHEN WALLET WAS LOST RECENTLY
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action. EHS HAS PROVIDED INFORMATION FOR ORS INTERACTIVE. OBTAIN NEW CARD IN 30 DAYS.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink at the BACK PREP AREA IS BEING USED TO STORE PLASTIC BAGS, UTENSILS AND OTHER ITEMS.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. DISCUSSED WITH OWNER , ALL ITEMS WERE REMOVED.
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site)
    Observation: Observed that paper towels were not provided at each handwashing sink. OBSERVED NO PAPER TOWELS IN THE HAND WASH SINKS IN THE KITCHEN AND IN THE UNISEX BATHROOM IN FRONT SITTING AREA.
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.DISCUSSED WITH MANAGER AND OWNER, PAPER TOWELS PROVIDED.

A ROUTINE INSPECTION of your facility was conducted today. Thank you for accompanying me during this visit as your participation allows me to clarify information and identify areas where processes may need further attention or assessment .
EHS discussed further with manager Certified Food Manager card requirements from ORS ,hand wash sign placements ( where required) . EHS has provided information to manager and owner of ORS location and contact information.

  • Handwashing Signage at Handwashing Facilities Required (corrected on site)
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees. OBSERVED THAT THERE WAS NO HAND WASHING SIGN AT ANY OF THE HADN SINKS USED BY EMPLOYEES.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. EHS PROVIDED HAND WASH SIGNS TO THE OWNER.
  • Lighting Intensity/ Walk-ins & Dry Storage/ 10 foot candles (corrected on site)
    Observation: Observed that inadequate lighting was provided in the walk-in refrigerators, walk-in freezers, and/or dry storage areas. OBSERVED THAT THE LIGHT BULB WAS NOT WORKING AND THERE WAS NO LIGHT IN THE WALK IN REFRIGERATOR
    Correction: Increase the lighting to provide a minimum of 10 foot candles at a distance of 30 inches off of the floor in these areas. MANAGER REPLACED THE LIGHT BULB IN THE WALK IN REFRIGERATOR.

  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers
    Observation: A food employee was observed eating in an area other than designated break area while preparing/handling food at the food preparation table.
    Correction: Food employees may only eat, drink, or use tobacco in a designated area like a dining room or office area. Employees shall not eat, drink, or use tobacco in the kitchen areas especially while preparing food for customers.
  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employe were observed handling the following ready-to-eat food using their bare hands
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.
  • Handwashing Sink / Accessible at All Times
    Observation: The handwashing facility located at the front serving line area is blocked with container stored in it , preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.

PREOPERATIONAL FINAL INSPECTION
1. Paper towel dispensers have not been installed at five hand washing sinks.
2. Soap dispensers have not been installed at five hand washing sinks.
3. A self-close device has not been installed at the rest room door.
4. Smooth and washable ceiling tiles have not been installed at employee's rest room.
5 Waste water plumbing drain lines between the three-compartment sink and the grease interceptor are not installed in accordance to the health department requirements. Wash and rinse compartment to be plumbed into the grease interceptor with adequate air-gaps.
6. Sandwich refrigerator is not working. CORRECTED.
7. The left and right of walk-in refrigerator/freezer have not been sealed to walls.
8. The reach-in freezer door handle is broken.
9. Shelves for walk-in refrigerator and walk-in freezer have not been installed.
10. A mop rack has not been installed at the mop rack.
11. Employees' lockers/clothes' hooks have not been provided.
12. Food Manager has not obtained CFM picture I.D. card.
*Owner shall correct the above listed items and schedule for a final follow up Health Department inspection.
