- Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution.
Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine. - Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.

Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at in the womens' bathroom used by food employees.
Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.

The purpose of today's visit was to conduct a risk factor assessment inspection. The CFM has agreed to email or fax confirmation to the EHS of successfully passing the CFM exam by the October 31, 2014. The Taste of Fall special menu also needs to be corrected/updated to include a complete consumer advisory disclosure and reminder statement by October 31, 2014.
- Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
Observation: A review of the Taste of Fall special menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked:---------two fresh eggs cooked any style.
Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.

The purpose of today's visit was to conduct a complaint investigation. A complaint was received by the Health Department on May 28, 2014. The complainant alleges that during a visit to the facility on Sunday, May 25 around 11:00 am that a cockroach was observed running across the dining room floor. Also complainant alleges that there was a hair on the plate when it was served at the table. EHS discussed complaint with manager on duty during today's inspection and manager was aware of complaint. Per manager that bug observed on May 25, 2014 was not a cockroach, but a water beetle. Manager shared that water beetles are coming from outside of the front door area. Pest control service is received monthly or as needed. The last service was received on April 20, 2014. EHS observed proper hair restraints being used by employees. Complaint cannot be confirmed.
