  • Equipment - Good Repair and Proper Adjustment
    Observation: Gap observed in sandwich unit door on right side.
    Correction: Repair the sandwich unit door to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the sandwich unit door, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Observed ice buildup in small fridge and freezer box in back area,
    Correction: Clean and sanitize these surfaces for food contact.

Spoke with certified food manager concerning correct test strips needed and proper hair restraints to be worn while working with food. Monitor sandwich unit door on right side for air flow. Manager to fill out application and send in permit fee of $40. Facility recommended for permit. Food permit will be issued once application and permit fee is received.

    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical (Qac) sanitizing solution.
    Correction: Obtain a chemical test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Lighting, Intensity (repeated violation)
    Observation: Less than 20 foot candles of light was noted in back area near walk-in cooler and ice machine.
    Correction: Provide at least 20 foot candles at a distance of 30 inches from the floor in consumer self-service areas, inside equipment, warewashing, equipment storage, toilet rooms

Spoke with manager about food temperature charts. Maintain temp chart daily. Copy of online food handler training information given to manager.

    Observation: Observed opening on door gasket of the sandwich unit.
    Correction: Repair or replace the sandwich unit door gasket in accordance with the manufacturer's specifications.

    Observation: Front panel on walk-in cooler was observed in a state of disrepair and damaged. Front panel loose.
    Correction: Repair the walk-in cooler front panel to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the front panel, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Light bulbs out in back room near walk-in cooler.
    Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor

    Correction: Remove ice buildup and clean and sanitize these surfaces for food contact.

Informed manager to maintain copies of all employee food handler cards on file at establishment. A food handler class schedule was left on site with certified food manager. Spoke with certified food manager about FOG program. Informed by certified food manager that certification was obtained and will bring certification in to post.

    Correction: Maintain accurate temperature logs.

Temperature logs are being recorded and maintained current. Spoke with manager about certified food manager certificate and having Serv Safe certificate transferred over to Norfolk certificate. Spoke with certified food manager about application and permit fee for Health permit. Observed good personal hygiene practices by staff. Overall facility is clean. Facility recommended for permit renewal. Informed manager once application and permit fee is received and processed, Health permit will be issued.
  • Ventilation Hood Systems; Filters
    Observation: Observed buildup on grease extraction filters under hood system.
    Correction: Alter or replace this equipment so that the filters can be easily removed for cleaning and replacement. Clean and sanitize hood filters.
  • Lighting, Intensity
    Observation: Less than 50 foot candles of light was noted under hood system.
    Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor
