Observed employees washing hands prior to preparing pizzas and using utensils after the pizzas were prepared.

Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

Observation: Vinyl coving was peeling away from the wall under the hand sink near the three compartment sink and was behind patched with duct tape in other areas. Also observed a few floor tiles under the three compartment sink to be chipped and in disrepair.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

Discussed the following items with the Person in Charge:
1. The pizza prep unit has not been replaced since the previous inspection. No food is stored in the bottom of the unit, and the compressor running for the top of the unit is still functioning properly. Foods all maintained at 41'F and below. Walk in cooler is used for all food storage other than the top of the pizza prep unit.
2. Plans will be required for upcoming remodel. Ensure plans are submitted to the health department with adequate time for review. EHS's contact information was left with the Person in Charge.

Observation: The following areas are not maintained in good repair: 1. Hole in the wall near the pizza prep cooler, 2. Coving peeling away from the wall all around the three compartment sink, 3. Floor tiles missing in the warewashing area, 4. The door handle to the rear door of the facility is broken.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Mops - Drying Mops (corrected on site)
Observation: Mops not hung up to air dry.
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

Observation: Containers of glue, all purpose spray and class cleaner are not properly stored to prevent the contamination of equipment and single service items.
Correction: Containers of cleaning chemicals must be located in an area that is not above food, equipment, utensils, linens or single service items.

Discussed the following with the Person in Charge:
1. The bottom of the pizza prep unit is currently not functioning properly. No foods are being stored in this portion of the unit. The top and bottom of the pizza prep are operating on separate condenser units. A new, complete unit has been ordered for this facility.
2. PIC stated the store was scheduled for a remodel of some sort this spring. Please contact the health department to assess whether a plan review is necessary based on the extent of the remodel.
- Handwashing Lavatory, Water Temperature, and Flow
Observation: Water from the handwashing sink next to the three compartment sink was measured at a temperature less than 100°F. The knob for hot water at this sink is currently not functioning properly, and hot water is not available.
Correction: Make necessary repairs to the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.

Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness.
