Observation: The following raw and/or undercooked foods are provided on the menu without proper disclosure: for cooked to served hamburger
Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods as raw/undercooked or identification of the animal-derived foods by asteriking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.

- Cooling Methods (corrected on site)
Observation: The following methods used for cooling were not adequate to facilitate proper cooling: Large container of marinara sauce was sitting on the counter top.
Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans
- Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3-vat with a concentration of 50 ppm total quaternary ammonium compound. Quat dispenser was not working correctly.
Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit. Could not correct 3-vat quat since quat chemical bottle could not be opened. Manager will contact maintenance right away to get this corrected. In the mean time, all utensils will be washed via dish machine.
- Window & Doors Screened if Kept Open for Ventilation
Observation: Back door was kept open for ventilation.
Correction: Keep door closed or install full screen door to prevent entrance of pests.
- Handwashing Signage at Handwashing Facilities Required (corrected on site)
Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees: handsink at pizza making station and men's and women's restrooms.
Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. Signs were posted.

The purpose of this visit is to conduct a First Risk Factor assessment. Manager demonstrated considerable active managerial control. Topics and handouts including extra handwashing signs, process temperatures and an extra employee health policy were provided to the manager. There were no critical violations observed during today's visit. (See note in file regarding consumer Advisory)
