    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

Today’s visit was to conduct a Risk Factor Assessment inspection.
During inspection, routine violation was found. The citation 4-501.11 (A): Equipment/Good Repair/Operation. The following refrigeration unit is not operationg properly and is unable to maintain cold food at 41F and below: walkin cooler at 56F and the 3dr preptop cooler at 68F along cookline. CFM and food employees removed all potentially hazardous foods in both units and stored food in freezer and cooler units.
Corrective Action: Repair/adjust unit(s) so it is able to maintain foods at or below 41F. Do not use the unit(s) to store potentially hazardous foods while it is not operating properly.
Do not use the walkin cooler and the 3dr preptop cooler until they are both repaired and are able to hold at 41F and below. Please fax to my atttention the invoice by 4/17/13 (Wednesday).

    Observation: The following cooked potentially hazardous foods are not being adequately cooled from 135°F to 70°F within 2 hours: wonton soup 112F under hotline for 3hrs and fried chicken 80F for 3hrs. CFM and food employee discarded items.
    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within 2 hours to prevent the growth of harmful bacteria.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: a)raw chicken 48F, raw scallops 59F, cooked pork 61F, raw pork 59F, and raw egg mix 58F in 3dr preptop cooler. b) raw chicken 52F, raw beef 50F, raw shrimp 49F, bean sprouts 54F, and tofu 48F in walkin cooler. All potentially hazardous foods were removed from the units and were stored in different coolers and freezers in facility since food was prepared on the same day.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: roasted pork, cooked pork, cooked duck in bev air 2dr cooler. CFM labeled items.
    Correction: Refrigerated, ready-to-eat, potentially hazardous food (time/temperature control for safety foods) that combined with additional ingredients, or portions of the same food, must retain the date marking of the earliest or first prepared ingredient.

    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Observation: Observed buckets of soy sauce stored on the floor and/or food stored less than 6 inches off the floor.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.

    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: chicken at 44F, and shrimp at 44F in 3DR prep unit.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. 3DR PREP UNIT WAS ADJUSTED TO COLD-HOLD FOODS AT 41F OR BELOW.

    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: flat-top cooler, 3DR prep unit.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Cooling, Heating, and Holding Capacities (corrected on site)
    Observation: The following unit was not meeting the cold-holding capabilities of the establishment by keeping foods at 41F or below.
    Correction: Equipment for cooling and heating food, and holding cold and hot food shall operate properly and be sufficient in number and storage capacity to provide food temperatures as specified under Chapter 3. THE PERSON IN CHARGE WAS INFORMED ABOUT THIS, AND ADJUSTED THE TEMPERATURE TO HOLD FOODS AT 41F OR BELOW.

    Observation: The handwashing sink located by the hood is blocked, preventing access by employees for easy handwashing. Additionally, the handwashing sink located by the water heater was blocked by a large rice container.
