Today a ROUTINE INSPECTION of your facility was conducted. Thank you for accompanying me during the inspection as your participation allows me to assess areas and processes at your facility that may require further clarification. A paper report was left at the time of inspection and this is the complete printed inspection report that accompanies your paper report. NOTE: At this inspection additional time was taken to review a Complete Consumer Advisory with the manager as the notice posted was missing the Consumer Disclosure statement. - Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: OBSERVED COOKED CHICKEN STORED IN THE SAME SHELF WITH RAW SHELL EGGS AND RAW BEEF IN THE TRUE 2 DR UPRIGHT REFRIGERATOR.
Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. DISCUSSED THE PUBLIC HEALTH PRINCIPLES BEHIND PROPER ARRANGEMENT AND PROTECION OF FOOD WITH THE PERSON IN CHARGE. THE COOKED CHICKEN WAS MOVED FROM THE UNIT INTO THE 2 DR FREEZER.

Observation: The raw and/or undercooked foods provided on the menu, menu board, table tent or brochure do not provide a disclosure statement.THE CONSUMER ADVISORY STAEMENT POSTED ON THE WALL ABOVE THE PREP UNIT DID NOT IDENTIFY THE FOOD ( EGGS AND HAMBURGER) THAT IS BEING OFFERED UNDERCOOKED /COOKED TO ORDER AND DID NOT HAVE THE DISCLOSURE STATEMENT. IT ONLY CONTAINED THE CONSUMER REMINDER STATEMENT.
Correction: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods as raw/undercooked or identification of the animal-derived foods by asterisking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients. DISCUSSED WITH THE PERSONS IN CHARGE TO IDENTIFY THE FOODS BEING OFFERED AS UNDERCOOKED AND WRITE IN THE CONSUMER REMINDER STATEMENT WHICH WAS DONE DURING INSPECTION.
