The purpose of this visit is to conduct a routine inspection. Provide copies of the update certified food manager card within 10 business days. Replace the ceiling tiles in the kitchen with the non-absorbent ceiling tiles in dry storage room.
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.
- Wall / Ceiling Covering / Nonabsorbent & Cleanable
Observation: Observed a few ceiling tiles in the kitchen by the hood that are absorbent and not easily cleanable.
Correction: Remove the absorbent ceiling tiles and replace with nonabsorbent easily cleanable tiles that are in the dry storage room.

Observation: Food is subject to environmental sources of contamination during preparation. OBSERVED PREPARED FOOD BEING STORED ON A THE PREPARATION TABLE WITHOUT A COVER WHILE PERSONEL NOT ESSENTIAL TO THE KITCHEN WALKED BY THE TABLE.
Correction: COVER FOOD WITH A PLASTIC WRAP OR OTHER MEANS TO PREVENT POSSIBLE ENVIRONMENTAL CONTAMINATION. During preparation, unpackaged food shall be protected from environmental sources of contamination. EMPLOYEES PLACED A COVER OVER THE FOOD.

The purpose of today's visit was to conduct a routine inspection. Observed facility clean and organized. EHS provided manager with and discussed the following handouts: Employee Health handouts (red folder) and CFM guidelines. Please fax or email a copy of successful exam certificate or CFM photo card no later than 30 calendar days since both CFMs cards expire in one week. Bingo food service is on Tuesday and Saturday evenings between 6:00 - 9:00 pm.

Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
Correction: A complete employee health policy must have the following elements:
1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

Observation: The food temperature measuring device l is not accurate in °F:-----observed that the full range food thermometer read below 0 F in ice water.
Correction: Food temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/- 2°F in the intended range of use. EHS PROVIDED FACILITY WITH NEW FULL RANGE THERMOMETER.

The purpose of this visit was to conduct a routine risk evaluation. Dishmachine turned thermolabel black. The food service establishment is not in compliance with having an Employee Health Policy. The Person-In-Charge is to provide training on the Employee Health Policy within 14 calendar days. Documentation (meeting attendance sheet/signed agreement form) is to be submitted to the Health Department within 14 days by fax or email that training has been completed. NOTE: PLEASE REMEMBER TO PROVIDE INVOICE FOR 2 DR UPRIGHT COOLER TO HEALTH DEPT. WITHIN TEN DAYS, and PROVIDE SIGNED EMPLOYEE REPORTING AGREEMENTS TO HEALTH DEPT. WITHIN 14 DAYS AS REQUIRED ABOVE.

Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Victory 2 dr upright left.
Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. NOTE: PIC will send copy of service invoice to health department wihtin 10 days. Cooler will not be used for the storage of potentially hazardous foods until it is restored to fully functional condition.
