Generally, facility observed to be clean and well ordered. Observed good active managerial controls in place. Good food temperatures observed throughout the flow of food. Strrong on knowledge of employee health policy but manager could not produce a written policy and no on-going training. Good handwashing procedures observed.
- Food Storage - Clean and Dry Location
Observation: Flouresent lighting tubes stored stacked against beverage syrup dispensing rack. Flourescent lighting tubes are considered hazmat due to the content of mercury and should not be stored in food storage/prep areas.
Correction: Remove light tubes and store with other hazmat awaiting disposal.
- Sewage - Other Liquid Wastes and Rainwater (repeated violation)
Observation: Frozen condensate observed around door to walk in freezer.
Correction: Repair door and/or replace gasket to ensure condensate either drains or evaporates from unit. Operator to develop plan of action to repair/replace within 2 weeks. Operator to send written plan to 434-972-4310 within one week by COB on Wed., March 25, 2015.
- Wall and Ceiling Coverings and Coatings
Observation: Ceiling panel in dry storage area ajar. Mnager states that the ceiling panel is kept that way to prevent drink lines from freezing during the winter.
Correction: Replace ceiling tile, discuss freezing issue with management.
- Physical Facilities in Good Repair
Observation: Ceiling above hot water heater in dry stores area is leaking/dripping on the hot water heater housing, and boxes of paper towels.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Handwashing Lavatories - Maintaining and Using Handwashing Lavatories
Observation: Women's restroom sink needs to be resealed along wall junction.
Correction: Keep handwashing facilities clean and maintained to encourage proper handwashing.

- Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
Observation: The nonfood contact surface of the cover of ice bin at self-service soda dispenser and lid to chest freezer for hamburgers are not corrosion resistant, nonabsorbent, and/or smooth.
Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
- Sewage - Other Liquid Wastes and Rainwater (repeated violation)
Observation: A condensate drain line servicing air handler in ceiling was emptying liquid waste directly onto the floor of the kitchen. Also, condensate freezing around door to walk in freezer
Correction: Repair/Relocate the condensate drain line to prevent liquid waste discharge onto the floor surface. Also repair walk in freezer so all condensate either drains or evaporates from unit. Listeria can be spread through condensate to the food. Operator to send an invoice of repair to health department on both of these corrections with date to repair or replace units by Close of Business Friday, April 11, 2014.
- Outer Openings - Protected (repeated violation)
Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Gap at bottom of rear door to outside can allow fly entry.
Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
