| LEx 1128001 A complaint was received that indicated that mice are present in the cafeteria at this facility. EHS reviewed facility pest control log. The last technician visit was on 08/19. Since then 3 ant sightings were recorded (08/26), 1 skunk was observed near the trailers on 9/1 and one request for 50-100 mousetraps was recorded on 09/02. Around the perimeter of the cafeteria EHS observed multiple openings such as weather stripping in need of replacement, apparent holes in the floor and a hole/gap in the wall. In addition EHS also observed ants, spiders and a few mouse droppings. Food debris was observed on some tables and floor that appeared to be leftover from breakfast. During the complaint inspection dated January 8, 2014 the school was notified in writing to repair/replace the damaged tiles. Please make arrangements for pest control service to be completed this calendar week and provide a copy of the report to EHS. Arrange for weather stripping and damaged tiles to be replaced within 30 days. This is the third complaint on this topic (January 8, 2014 and October 22, 2014) please follow-up with staff as needed. Note: the kitchen is noticeably cleaner than other areas of the school that were observed today and is not contributing to any pest issues. | ACR 37277 A complaint was received that stated that a student observed rodent and insect activity in the school on the same day last week. EHS made a site visit and did not observe any evidence of rodent activity in the kitchen. There were multiple glueboards dated 10/6 in the kitchen and they were observed to be mostly clear except for very light spider and adult roach activity. The kitchen of this facility is in overall clean condition and is not providing harborage conditions or contributing to any pest issues that the school may be experiencing. A copy of the ACR compliant report was provided to Ass't Principal Kezmarsky. Please note that this is the second complaint that the Health Department has received on this topic during the 2014 calendar year. School management is advised to more aggressively address this issue with school custodial staff and any pest control/IPM vendors. | ACR 32659 Site visit made to complete complaint inspection while cafeteria not in use. EHS met with Head Custodian and walked the perimeter of the cafeteria. Some damaged floor tiles were observed along the wall opposite the serving lines. EHS advised the Head Custodian to repair/replace the tiles that have broken to expose the sub-floor. No rodents or rodent droppings were observed. Pumpkin seed debris was observed to the right of the microwave. Head Custodian indicated that he will instruct staff to clean this area and to be more careful in the future. EHS asked PIC (Head Custodian) to be sure to monitor the room for activity in the future. Please review glove use, how to monitor reheat temperatures and date marking with staff. Observed thermometers being calibrated - thank you.
