The purpose of today's visit was to conduct a risk-factor assessment. The following suggestions have been made as a means to incorporate Active Managerial Control: 1. Immediately review Employee Health Policy documents provided (English, Spanish) with all new and long term staff 2. Train all employees using "How to Prevent Cross Contamination" document provided today (English, Spanish) 3. Create a cleaning schedule and log for all food and non-food contact surfaces as a means of preventative pest control measures
- Duties / Inform Employees about Responsibility to Report GI Symptoms, Disease, Medical Condition or Exposure to PIC (corrected on site)
Observation: Employees are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food. WHEN PROMPTED, PERSON IN CHARGE WAS UNABLE TO DESCRIBE CURRENT HEALTH POLICY IN PLACE.
Correction: The Person in Charge or certified food manager shall inform all food employees and conditional employees of their responsibility to report any information about their health and activities as they relate to diseases that are transmissable through food as specified under 2-201.11(A). PROVIDED EMPLOYEE HEALTH POLICY TRAINING.

Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: IN CARRIER 1DR UPRIGHT COOLER- OBSERVED RAW SHELL EGGS AND RAW CHICKEN BEING STORED OVER READY TO EAT TAMALES.
Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. TAMALES WERE PLACED ON SHELVING UNIT ABOVE RAW CHICKEN AND EGGS.
- Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: INTERIOR OF CARRIER 1 DR COOLER, WALLS AND FLOORS NEAR GREASE TRAP.
Correction: PLEASE REGULARLY CLEAN INTERIOR OF COOLERS AND FLOORS/WALLS OF PREP AREAS TO AVOID AN ACCUMULATION OF FOOD AND DEBRIS. Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
