    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Do not temperature abuse pizza sauce by leaving on the make table for more than 5 minutes. Shelf stable canned pizza sauce ingredients can be refrigerated prior to combining to result in a product below 41F.
  • Equipment and Utensils - Multiuse, Characteristics (repeated violation)
    Observation: The food contact surface of the toppings containers is not durable, nonabsorbent, easily cleanable, resistant to pitting.
    Correction: Replace the toppings containers to permit easy cleaning and prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food.
  • Temperature - Food Temperature Measuring Devices - Provided
    Observation: The Person in Charge could not produce an appropriate food temperature measuring device to verify cooking temperatures of thin foods such as meat patties or fish filets.
    Correction: A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.

Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: The person in charge failed to state the required temperatures and holding times for the following conditions of potentially hazardous food: (1) Refrigerated storage, (2. time control procedures 3. wash/rinse/sanitizer procedure, 4. proper sanitizer concentration
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.

    Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
    Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever.     Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a ammonia test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Observation: Person in charge did not know the proper sanitizer concentration. Dough mixer was observed encrusted with dough from use approximately 5 hours prior.
    Correction: Adjust the sanitizing solution to 200 parts per million or otherwise identified by manufacturer's recommendations.

    Observation: The nonfood contact surface of the oven handles are covered by soiled towels.
    Correction: Remove the soiled towels from the oven handles to preclude accumulation of soil residues that become encrusted or attract insects and rodents and cross contaminate hands.

    Correction: Discontinue storage of clean equipment and utensils in a location subject to contamination.

This inspection form will serve as temporary permit to operate until annual permit can be obtained on Mon 3-25-13. Building official verbally expressed approval of construction and will fax Certificate of Occupancy to the Montgomery Health Department on Mon 3-25-13.
