Generally, facility observed to be clean and well ordered. Good food temperatures observed throughout the flow of food. Left handounts on employee health.
  • Plumbing System Maintained in Good Repair (repeated violation)
    Observation: Observed drainwater going on floor under 3-compartment sink. Also observed grease trap to be very small in relation to the size of the 3-compartment sink and open site drain had be added to middle of drain pipe under 3-compartment sink..
    Correction: Advised operator to have a commercial plumber evaluate current design and construction of open site drain and grease trap size as well as cleaning/maintenance schedule for grease trap. The goal being to ensure drainwater coming from sink does not go onto floor. Operator to send evaluation by plumber to health department by fax or email by COB on Monday, March 2, 2015.
  • Floors, Walls, and Ceilings - Cleanability (repeated violation)
    Observation: Ceiling in kitchen is staining and paint peeeling. It is not smooth and easily cleanable.
    Correction: Repair or replace ceiling to make it smooth and easily cleanable.

Generally, facility appeared to be clean and well ordered. Staff knowledgeable about employee health policy, good food temperatures observed throughout the flow of food. Sanitizer at 3 compartment sink at proper concentration-quaternary ammonia at 200ppm.
  • Plumbing System Maintained in Good Repair (repeated violation)
    Observation: Plumbing connections under the 3 compartment sink is leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Ceiling and walls in kitchen stained and peeling. Ceiling in window for drive through also has peeling surface. Cover over fan in ladies room is not attached. None of these areas are being maintained in good repair
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. operator has agreed to repaint and resurface walls and ceiling and repair fan cover in ladies restroom
  • Premises - Maintaining Premises - Unnecessary Items and Litter
    Observation: Premises has accumulation of litter around outside storage unit along with high weeds which can harbor pests.
    Correction: Maintain the premise free of litter and clear out weeds to minimize pest harborage and food sources..

Generally, facility appeared to be clean and well ordered. New process for cooling sliced deli meat in freezer prior to placing out for prep. Hot water at 3 compartment sink was high at 135oF Operator to check with chemical supplier to ensure proper water temperature for adequate sanitization in 3 compartment sink.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Small undercounter prep refrigerator was observed in a state of disrepair and damaged--it appears temperature is slightly elevated and it needs repair..
    Correction: Repair the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: Wall or wall covering behind 3-compartment sink is black, rough, and is not smooth and easily cleanable.
    Correction: Repair or replace wall or wall covering to make it smooth and easily cleanable.
  • Lighting, Intensity (repeated violation)
    Observation: Less than 50 foot candles of light was noted in the rear prep area at prep table where slicer located and under hood
    Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor
  • Lighting, Intensity (repeated violation)
    Observation: Less than 20 foot candles of light was noted in the front kitchen handwash area.
    Correction: Provide at least 20 foot candles at a distance of 30 inches from the floor in consumer self-service areas, inside equipment, warewashing, equipment storage, toilet rooms
