KDA_3_50118A1 - --REPEAT: There was a package of sliced turkey in the walk in cooler that was dated 11/12-11/19 and should have been discarded yesterday 10/18. COS- PIC voluntarily discarded.
KDA_3_50118A3 - --There was an opened package of ham in the walk in cooler that was dated 11/16-11/23. COS- PIC correctly date marked. There was a container of mashed potatoes in the walk in cooler that was dated 11/19 at 7pm to 11/25. PIC stated the mashed potatoes were made 11/18 at 7pm. COS- PIC correctly date marked.
KDA_4_50111A - --The True Freezer in the back prep area is broken and currently not in use. PIC stated the freezer is being sold. Walk in cooler #3 is currently down and not in use. PIC stated that the cooler will not be fixed and instead will be used as dry storage.
KDA_4_50213B - --There were two dispensing milk tubes on the self service island that were not cut at a diagonal.
KDA_4_60111A - --REPEAT: There was old milk residue build up inside of the milk dispensing unit.
KDA_4_70211 - --REPEAT: The hot water measured at 125.4F in the dishmachine after being ran two times. Dishes had already been washed. PIC called maintenance and set up 3 vat sink temporarily to clean dishes. All dishes were recleaned.
KDA_6_50112A - --There was a rancid smell around the milk dispensing unit in the lobby area.

KDA_3_10111 - --There was an opened package of sliced Jennie O turkey in walk in cooler #2 without any date marking. PIC was unable to identify prep date. COS-PIC voluntarily discarded.
KDA_3_30211A1a - --There was a package of raw turkey bacon stored directly over a whole watermelon in walk in cooler #1. COS- PIC moved the bacon to a different area.
KDA_3_50118A1 - --There was an opened package of deli ham in walk in cooler 2 that was dated 10/28. PIC stated the 10/28 was the prep date. COS- PIC voluntarily discarded. There was a container of pizza sauce dated 11/2 and a container of salsa dated 10/28 in walk in cooler #1. PIC stated those were the prep dates. COS- PIC voluntarily discarded both items.
KDA_3_50118A2 - --There were two opened gallons of soft serve mix (vanilla and chocolate) in the upright reach in cooler in the front without any date marking. PIC stated the gallons were opened Friday 11/6. COS- PIC correctly date marked.
KDA_4_50111A - --The True Freezer in the back prep area is broken and currently not in use. PIC was unsure if it was going to fixed or not. Walk in cooler #3 is currently down and not in use. PIC stated parts have been ordered.
KDA_4_60111A - --There was old food debris on the blade of the commercial slicer in the back prep area. There was old milk residue build up inside of the self service milk dispensing unit.
KDA_4_70211 - --The hot water measured at 137.1F in the dishmachine after being ran three times. Dishes had already been washed. PIC called maintenance and set up 3 vat sink temporarily to clean dishes.
KDA_5_40213 - --There was back up sewage in the drain under the milk/cereal self service station in the lobby area.
KDA_5_50116B - --There was no trash receptacle available at the handsink nearest to the 3 vat sink.
KDA_6_50112A - --There was a rancid smell around the milk dispensing unit in the lobby area.
