  • KDA_2_40111 - --Sitting on the make table, an ash trash with cigarette butts in it.
  • KDA_3_10111 - --In the make table, there was a half of tomato with out a date mark. Date was not know when it was sliced. COS-tomato was discarded.
  • KDA_3_30212 - --Sitting on a shelf beside the reach in freezer, was a clear container with a white substance in it, with out a common name on it. Sitting in a metal shelf above the cutting boards, was a squeeze bottle with a yellow liquid in it with out a common name.
  • KDA_3_50116A2 - --In the make table, a half of tomato was 44.1F. Employee said it had been in there since yesterday. COS-tomato was discarded. Ambient of make table was 44.8F. Educated on sitting PHF on ice.
  • KDA_3_50118A1 - --In the walk in cooler, there was a half of head of lettuce that was dated 9/5/15. It was held longer then 7 days. COS-lettuce was discarded.
  • KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14.
  • KDA_5_20511B - --The hand washing sink in the kitchen, had a garden hose in it and a piece of cardboard over the hose. COS- items were removed from hand washing sink.
  • KDA_6_20214 - --The men's restroom does not have a self closing door.
  • KDA_6_20215A3 - --There is a 1/4" gap along the bottom edge of the front door.
  • KDA_6_50111 - --There are stained tile through out the kitchen. In the customer area, the vents that are painted black have paint peeling off them.
  • KDA_6_50112A - --The ceiling fans have an accumulation of dust. In the kitchen there are two free standing fans that have an accumulation of dust.
  • KDA_Defacto - --In the reach in make table: raw hamburger patties(they were only PHF in there)41.6. Ambient of reach in make table 41.7F. In the walk in cooler a head of lettuce was 38.5F. --On original inspection I listed free standing fans under here.

  • KDA_2_30115 - --Employee washed hands at the three compartment sink in the kitchen. Educated on handwashing. COS-Employee washed hands at handwashing sink.
  • KDA_2_40111 - --Sitting on the microwave was an ashtray with cigarette butts and ashes in it.
  • KDA_3_10111 - --On a shelf above the make table, there were three buns that had green mold. In the make table, there was a 1/2 tomato that had black spots on it, cheese slices had black spots on them. In the reach in make table, there was white sliced cheese that had black mold on it. In the walk in cooler, there was a head of lettuce that was green and 5 whole tomatoes that had black spots on them. COS-these items were discarded. In the bar area glass reach in cooler, there was an opened gallon of milk without a date mark. The best buy date was 8/9/15. Employee had no idea when it was opened. COS-Employee discarded the milk.
  • KDA_3_30212 - --In the left cabinet in the kitchen, there is a container with a white substance that is without a common name.
  • KDA_3_50116A2 - --Right side of make table: slice cheese 45.4F, sliced tomato 45.7F and lettuce 45.1F. Ambient of make table was 44.6F. These were the only PHF's in the make table. COS- Employee discarded these items. Reach in make table: sliced cheese 43.3F. Ambient of reach in was 40.8F. COS- Employee discarded cheese.
  • KDA_3_50118A2 - --In the make table, there was a container of sliced tomatoes that did not have a date. In the bar reach in a gallon of opened milk was without a date. Employee did not know when these were opened. COS-Employee discarded these items.
  • KDA_4_20211A2 - --In the right kitchen cabinet, there were three containers that were pitted. Inspector left them out on the counter.
  • KDA_4_30111 - --The make table ambient was not holding PHF at 41F. Ambient was 44.6F.
  • KDA_4_60111A - --On a shelf in the kitchen, stored as clean were three metal pans that had food debris in them.
  • KDA_4_60111C - --There are two free standing fans in the kitchen that have an accumulation of dust.
  • KDA_4_60212B - --The inside of the microwave has an accumulation of dried food debris on the door and the cavity.
  • KDA_4_90311B - --There are two containers of serving utensils above the make table that are not stored inverted.
  • KDA_5_20511A - --The handwashing sink in the bar area had two chemical bottles in the basin of it. COS-Employee removed these items.
  • KDA_6_20214 - --The men's restroom does not have a self closing door.
  • KDA_6_20215A3 - --There is about 1/4" gap along the front door.
  • KDA_6_30112 - --The paper towel machine in the kitchen was not working properly for inspector to get towels.
  • KDA_6_50111 - --The vents in the customer area have paint peeling off of them. There are stained tile through out the kitchen.
  • KDA_6_50112A - --In the corner of the four compartment sink, there is cobwebs. The window ledges have dust and small dead bugs. There is water on the floor of the walk in cooler. The ceiling fans have an accumulation of dust on them.
  • KDA_6_50116 - --The wet mop was stored in the mop bucket with soiled water in it.
  • KDA_6_50119 - --The men's restroom door was opened during business hours. COS-Inspector shut door.
  • KDA_Defacto - --Frozen food is frozen. --Reach in make table: sliced cheese 43F. Walk in cooler ambient was 39.8F. --The four compartment sink in the bar had 100PPM chlorine. --The owner or cook was not working at the time the inspection was conducted.
