Employee removed the contaminated gloves and put on a new pair of gloves without a hand wash in between tasks. COS education KDA_3_50116A1 - --Made on site white gravy in steam table top at 118F. PIC stated it was made maybe 2 hours prior. COS added water to steam table top. KDA_4_60211E4 - --Interior of ice bin and divider place of ice machine soiled with mold and rust buildup. Rust appears to be in direct contact with the ice on the side of the interior of machine. KDA_4_60213 - --Underside of grill area is soiled with food debris buildup and littered with stray pans. Interior of reach in cooler under make table top soiled with food debris buildup. Interior of reach in cooler to right of grill soiled with food debris buildup. KDA_5_20212A - --Handsink in women's toilet room at 59F-61F. Handsink did not reach 100F. KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14. KDA_5_20511B - --Handsink in front prep area used to fill a sanitizer bucket as well as fill a metal pan to go into the steam table top. COS education KDA_5_20515A - --Two leaks in establishment. One leak under soda fountain area in front of house with mold growing on tile under soda fountain cabinet. Another leak in back kitchen area by handsink and where towels are stored. PIC stated she has been trying to get corporate to fix the leaks. Both leaks are causing mold to grow in both locations in the restaurant. The cold water is turned off in the back kitchen at the prep sink, the 3 vat sink, and the handsink due to this leak. The faucet is leaking at the 3 vat sink in back kitchen as well. KDA_6_30114 - --No handwashing signage at handsinks in toilet rooms. COS supplied KDA_6_50112A - --Food debris and dirt buildup on walls and floor in dry storage area. Mold buildup on vent in kitchen area. KDA_6_50119 - --Door to men's toilet room does not close completely and leaves a 1/2" gap. KDA_7_10211 - --3 containers of blue liquid on drainboard under service counter with utensils in liquid and no label on container to state contents. PIC stated it was soapy water. COS labeled. Metal pans hanging on wall on opposite sides of service counter with clear liquid in pans, some with rags, some without, without label to state contents of pan. PIC stated it was sanitizer.
