KDA_3_30212 - --In back storage area there was containers not labeled and not easily identifiable. PIC stated it was flour and sugar. COS labeled KDA_3_50116A2 - --In back room cookie dough stored on shelf. PIC stated she had placed it there an hour before to soften. Peanut butter dough 73.5, monster cookie dough 73 and chocolate chip cookie dough 53.5. COS- PIC began the cooking process In grill area there was sliced cheese at 54.9F. PIC stated it had been pulled at 7:00 a.m. COS- place bag of ice over cheese. In grill area there was sliced RTE turkey bacon at 62F. Package stated fully cooked and must refrigerate. PIC stated it had been pulled at 7:00 a.m. COS- place bag of ice over bacon. KDA_3_50118A2 - --In RIC there was ham and roast beef with no date. PIC stated it was opened 10/30. COS- dated In RIC there was a container of corned beef dated 10/15. PIC stated it was no updated with the correct date of 10/30. COS- dated In RIC there was 2 packages of hotdogs not dated. PIC stated they were opened 10/29. COS-dated KDA_3_50118A3 - --In RIC there was a container of ham salad dated 10/27. Discard date was 11/2. COS-discarded KDA_4_20211A2 - --In back storage area there was a spatula stored as clean but was broken. COS- discarded In storage area there were several cracked containers. COS- discarded KDA_4_60111A - --In back room there was a tray of clean utensils stored on a tray with dried food debris. COS-removed for washing. KDA_4_60111C - --The fan guards in the WIC have and accumulation of dust. There is dried grease buildup on the hood filters located in the kitchen area. Observed by KDA 13 KDA_4_60211E4 - --in front kitchen area there is pink mold buildup in the inside ice deflector in ice machine. KDA_5_20515B - --The water pump attached to the espresso machine is leaking. KDA_6_10111A - --The wall in the dry storage area were food prep and dish washing is being done are not easily cleanable. Observed by KDA 13 KDA_6_30114 - --No signage at hand washing sink in back ware wash area. COS- supplied stickers. KDA_6_50111 - --The cupboard under the espresso machine is crumbling and has water damage. There are several water stains on the ceiling tiles in the front kitchen area, and the dry storage area. There is paint peeling on the wall behind the three vat sink. Observed by KDA 13 KDA_6_501111A - --In the cabinet under the cappuccino machine there were 10-15 fresh rodent droppings. KDA_7_20111B - --In back storage area there were 3 bottles of bleach stored over serving baskets. COS- moved bleach In front food prep area there was a bottle of glass cleaner stored over muffin tins.
