KDA_4_20211A2 - --There is a cracked pitcher being used to add hot water to the refried beans. Corrected on Site (COS) - spoke with the manager about this. A new pitcher has already been ordered and received. She removed the cracked pitcher and replaced it with the new one.
KDA_4_60111C - --There is slight dust/grease accumulation in the vent hood. There is a fan on the floor near the office that was running and blowing into the prep area upon beginning inspection. There is dust accumulation on the fan blades.
KDA_4_90311C - --On a shelf above the coffee machine there are single-use containers that are being stored outside of their original protective packaging, neither covered nor inverted.

Two separate staff members said it was made this morning. --Make Table: (No temperatures, items are under time as a control) Cooler under Make Table: Cheese - 41.4 F Walk-in-Cooler: Chicken - 38.4 F --Ownership verified. --Quaternary ammonia test strips provided. 200ppm quaternary ammonia being used in sink to sanitize dishes. --The make table with the toppings for the tacos, etc is under time as a control. The establishment has a posted on the wall behind the make table with stickers marking what time the items are to be discarded. --The soap dispenser in the men's restroom is hanging down/open. It is still dispensing soap when the top is pushed up and a hand is placed under the dispenser. --The wiping cloths are stored in 200ppm quaternary ammonia.
