- 3-302.11(A)(2) - --There were 2 plastic containers with raw ground beef being stored directly above a carton of raw hard shell eggs. COS-placed eggs at the bottom of the door frame. (Note: no leakage detected).
- KDA_3_30211A1a - --In the Frigidaire refrigerator there was a ball of raw hamburger being stored directly next to an open bag of ready to eat cut salad and a bag of ready to eat cut lettuce. COS-placed it with other raw items. (Note: the raw hamburger was in a food grade plastic bag, no leakage detected).
- KDA_3_30211A1b - --In the Frigidaire refrigerator there was an open package of raw bacon being stored directly next to an open bag of ready to eat shredded cheddar cheese and a closed tub of ready to eat margarine. COS-separated food. (Note: no leakage detected).
- KDA_3_50116A1 - --In a metal strainer to the left of the fryer there were fries at 82.4F. The person in charge said she discards them every 10 minutes if they are not used. She made them around 12:20pm and were tempted around 12:35pm. COS-discarded.
- KDA_3_50116A2 - --On the counter there were plastic food grade bowls of potato salad and coleslaw (both made on site) at 58.8F and 56.7F. The person in charge said she made them around 8am today and placed them in the Beverage-Air RIC until 11:45am. They were tempted around 12:42pm (during lunch rush-less than 4 hours). COS-placed the bowls back in the cooler when not using them. The ambient air temperature of the unit was reading 36.5F.
- KDA_4_10111A - --In the Frigidaire refrigerator there was a plastic non-food grade Sterilite container with sliced cheese in direct contact with the container. COS-placed aluminum foil around the cheese.
- KDA_4_20211A2 - --On a shelf above the Kitchen Aid mixer there were 2 rubber spatulas, one with a deep cut on the food contact surface (not easy to clean) and one with rough, cracked edges. COS-discarded one and trimmed the other. On the table in the ware wash area there was one rubber spatula with rough, cracked edges. COS-trimmed the edges smooth. In the storage room with the hot water tank, in a strainer there was a red plastic 1/2 cup that was broken. The PIC said it was used to portion the raw hamburger. COS-discarded.
- KDA_4_204120 - --The Beverage-Air RIC has water pooling at the bottom of the unit.
- KDA_4_50111B - --The seal around the release lever to the Taylor Ice Cream machine leaks. The PIC is aware and has ordered the seal to be replaced.
- KDA_4_60111A - --The Kitchen Aid mixer had dried food debris on the metal surface frame and stored clean. The Multimixer used for malts/shakes also had dried food debris on the metal frame and stored clean. Both of the mixers were used today. COS-the PIC cleaned both mixers. On a table in the ware wash area there was one round plastic lid with a sticker label left on it and stored clean. COS-moved to the ware wash area. In the storage room with the hot water tank, in a strainer there was one round plastic lid with a sticker label left on it and stored clean. COS-moved to the ware wash areal.
- KDA_4_60213 - --The handle on the Oster oven has oil residue on it.
- KDA_5_20515B - --The extended faucet connector at the 3 compartment sink leaks when water is supplied.
- KDA_6_20211A - --The light fixtures in the dry food storage room do not have protective covers. The light fixture in the Beverage-Air RIC does not have a protective cover.
- KDA_6_20214 - --The public restroom does not have a self-closing door and it opens into the ware wash area.
- KDA_6_50112A - --There is grease on the floor below the fryer. The tile floor in the dining room and kitchen is sticky. There is trash on the floor in the storage closet. There are empty boxes in the dry food storage room.
- KDA_7_20111A - --On a table in the dry food storage room there was a working spray bottle of Clorox bleach being stored directly next to a bag of toasted rice cereal, a bag of Aunt Jemima flour and a bag of Golden Dipt chicken fry powder. COS-removed the chemical.
