- KDA_Defacto - --A new AVB has been installed at the mop sink. --A new make table has been ordered to replace the older broken unit. --A new screen door and frame has been installed at the back door location. --A second sink has been added for drinking water only. There are now 2 sinks in the serving area. --All dishes that will fit inside the dish machine will be properly washed and sanitized. At the end of the day, the buckets and other large items not fitting inside the dish machine will be washed in the 3vat sink. Staff will remove the butcher block table, wash the dishes, and put the table back each day. --Dishes have all been cleaned and sanitized. A new make table has been purchased to replace the old table. --NOTE: The owner contacted KDA#19 and requested the follow up be conducted before the 10 days. --The kitchen area has been cleaned and walls and ceilings have been freshly painted. --There is now a designated area for all staff personal items.

- 5-202.14 - --There is a cloth rag taped completely around the AVB on the mop sink faucet.
- KDA_4_30112A - --The three compartment sink is not used at all in the FE. It is covered with a large butcher block cutting board. There are special pizza pans that are not placed inside the dish machine. PIC said they just wash and rinse them, then dry them to "Keep the seasoning" on the pans. These special pans are never sanitized and this is the food contact surface where the pizza dough is placed. There is a large sink in the kitchen that is directly plumbed and has a garbage disposal attached to it. It does not hold water. Dishes are washed and rinsed in the same sink. Produce is washed in the same sink. The butcher block table needs to be removed so that dishes that are not placed inside the dish machine can properly be washed, rinsed and sanitized.
- KDA_4_50111A - --The make table doors and lids are being held together with duct tape and they are not clean nor are they cleanable.
- KDA_4_60111A - --Located on the rack for clean dishes there was a whisk with grease and lent debris on the food contact surface. COS. Item was placed at dish area. The cutting board attached to the make table has an accumulation of black debris on the food contact surface.
- KDA_4_60111C - --The in use dough machine and mixer have a large accumulation of older aged crusty food debris. The back up dough machine has a large accumulation of dried food debris. Educated staff to clean machine and cover it with food approved plastic to keep it from getting dirty on the storage rack. There is a tub of papers and misc. on a shelf above the make table that has an accumulation of black debris on the handles and edges. The doors and door handles to the make table have an accumulation of sticky dried food debris. The WIC vent covers have an accumulation of greasy lent residue. The rack shelving above the butcher block preparation table has an accumulation of grease and lent debris. The roof of the ice machine has an accumulation of orange dirt debris.
- KDA_4_60313 - --The regular pizza pans were not loaded in the dish machine correctly so that they could be washed and sanitized correctly. They were stacked on top of each other. This complete load was washed during inspection.
- KDA_5_20511B - --Located in the front cashier area, there is a hand sink being used as a hand sink and a drinking water sink. The faucet has a water filter attached to it and they get glasses and pitchers of water from the same sink. There is soap and single use towels at the sink. FE will need to add an additional sink so that one sink can be a hand sink and one sink will be a drinking water sink.
- KDA_6_20215A3 - --The back screen door has gaps in the bottom and sides that are more than 1/4 inches.
- KDA_6_30114 - --There is not any signage at any of the hand sinks. Signage was provided.
- KDA_6_30511B - --There is a personal back pack sitting on the shelf and touching the containers of salt and corn meal.
- KDA_6_50112A - --The ceilings, venting and piping in the kitchen area have an accumulation of greasy lent debris.
- KDA_7_10211 - --There were 2 buckets with clear water and a wiping rag and no label on the bucket. PIC said both buckets were bleach water. COS. Buckets were labeled.
- KDA_7_20111A - --There was a roll of plastic wrap for food items sitting beside and touching 1 case of hand soap. COS. Plastic wrap was moved.

- 5-202.14 - A backflow or backsiphonage prevention DEVICE installed on a WATER SUPPLY system shall meet American Society of Sanitary Engineering (A.S.S.E.) STANDARDS for construction, installation, maintenance, inspection, and testing for that specific application and type of device.
- KDA_4_30112A - Except as specified in ¶ 4-301.12(C) and ¶ (F), a sink with at least 3 compartments shall be provided for manually washing, rinsing, and SANITIZING EQUIPMENT and UTENSILS.
- KDA_4_50111A - EQUIPMENT shall be maintained in a state of repair and condition that meets the requirements specified under Parts 4-1 and 4-2.
