  • KDA_4_20211A2 - --There were 3 rubber spatulas on the clean utensil wall with broken cracks and crevices along the food contact surfaces. COS- PIC voluntarily discarded.
  • KDA_4_60111A - --There were approximately 6 food storage containers, 1 food storage lid, 2 sauce bottles and 2 spatulas with old food debris along the food contact surfaces stored on the clean utensil shelf. COS- PIC moved all dirty equipment to the warewashing area for additional cleaning.
  • KDA_4_90311B - --There were several plastic food storage containers stacked together wet not allowed to properly air dry.
  • KDA_5_20515B - --The atmospheric vacuum breaker (AVB) on the mop sink faucet is leaking.
  • KDA_5_40213 - --There is back up sewage leaking out of the underground piping system for the grease trap outside behind the facility. After speaking with an individual at the Grease interceptor maintenance program that is contracted, I learned that the underground system has not been pumped since September of 2014. Individual stated that a plumbing issue may be the problem as well. No back up sewage inside the facility.
  • KDA_5_50117 - --There is no covered trash receptacle in the unisex employee restroom in the back.
  • KDA_6_30311A - --There is a light out in the walk in freezer.
  • KDA_6_501111A - --There was a live cockroach near the mop sink in the back. There is a pest control company is contracted, last visit was 9/18/15. Technician noted internal cockroach activity.
  • KDA_6_501112 - --There were approximately 5-10 dead cockroaches around the facility, primarily in the back storage area.
  • KDA_6_50112A - --There is grease, dirt, grime and old food debris built up throughout the facility. Recommended a deep cleaning.
  • KDA_Defacto - --Chlorine measured at 100ppm in warewashing unit, test strips provided. --meatballs 39 (reach in cooler) diced tomatoes 40 (make table) sausage 41 (walk in cooler) --pizza 194 (oven) --Wiping cloth QT measured at 200ppm.
