COS- labeled. KDA_3_30412B - --The handle of the scoop located in the sugar bin was in contact with the sugar. COS- moved handle above sugar. KDA_3_30511A1 - --Located in the dry foods storage room, there was a box of bananas that was being stored on the floor. KDA_4_20211A2 - --Located in a drawer under the counter in the kitchen area, there was one rubber spatula that had a crack in it. COS- educated PIC and removed from use. KDA_4_60111A - --Located in a plastic container on the counter next to the three vat sink, there were two tongs stored as clean that were soiled with dried food debris. Corrected on site (COS)- moved to wash area. Located on a storage rack between the walk in cooler (WIC) and walk in freezer (WIF), there was one cutting board stored as clean that was soiled with dried food debris. COS- moved to wash area. Located in the drawers under the counter in the kitchen, there were two tongs, three scoops, one ladle, one plastic spatula, and four rubber spatulas stored as clean that were soiled with dried food debris. There were also two plastic measuring cups that had sticker residue on them. COS- moved to wash area. The scoop that was located in the flour bin was soiled with dried food debris. COS- moved to wash area. Located on the shelf under the island counter in the kitchen, there was one metal pan stored as clean that was soiled with dried food debris. COS- moved to wash area. Located in the dry food storage room, there were six plastic containers stored as clean that were soiled with sticker residue. Most of the plastic lids in this area that were stored as clean were also soiled with sticker residue. There was also a plastic container stored as clean that still had part of a seal around the rim of the container. COS- moved to wash area. KDA_4_90311A - --Located in the storage area next to the cafeteria, there was a box of napkins that was being stored on a two by four on the floor. KDA_5_40211 - --There is a direct drain connection on the food preparation sink. Employees were washing eggs and tomatoes in the sink below the flood rim of the sink. KDA_7_10211 - --Located in the chemical storage room, there was an unlabeled container of green liquid which the person in charge (PIC) identified as delimer. COS- labeled.
