KDA_3_30414B1 - --The wiping cloth bucket near the hand sink in the back is showing 0ppm sanitizer. It was tested for chlorine and quaternary ammonia as the establishment has both available.
KDA_3_30511A1 - --In the walk-in-freezer there are boxes of hot cakes and mcgriddles stored directly on the floor.
KDA_3_50116A2 - --There is a container of canadian bacon stored on top of the cooler by the grill. It is measuring 54.0 F. The cook said that it had not been sitting out very long as he had to go get it from the walk-in and had gotten busy and stepped away from it. COS - it was returned to the cooler. There is a tub of apple slices and gogurt by the window register. The gogurt is measuring 62.7 F. An employee said it had been there for approximately 40 minutes. COS - the tub was put into the cooler.
KDA_3_50119B2 - --At the prep line there are sliced tomatoes, sliced cheese, shredded cheese, and southwest vegetables out under time as a control. There are no time stamps for any of these containers. A staff member identified when they were all put out within the past 3 hours. COS - all containers were marked with the appropriate time of when they were last filled.
KDA_4_20211A2 - --There is a cracked pitcher on a shelf near the prep area. There is a chipped and cracked food container on a shelf near the prep area. The lid to the salad container in the walk-in-cooler is chipped along the corner. The container holding the gogurt and sliced apples is chipped on the corner. COS - all containers were removed from use and taken to the office to be reordered.
KDA_4_50213B - --The cream dispenser near the window register has the tube sticking out of the dispenser 2-3 inches. The tube is not cut on the diagonal.
KDA_4_60111A - --On a shelf of clean dishes there is a lid with a sticker remaining on the outside. Corrected on Site (COS) - the assistant manager moved this to ware washing. The tub of apples below the hot cases has a sticker on the outside of the tub. The sticker is the manufacturer's label remaining from the original purchasing of the container. COS - this is being moved to ware washing. The lemon slicer has food residue remaining on the blades of the slicer. COS - had this moved to ware washing. There is fallen debris on a piece of parchment paper in a box of parchment paper. COS - the top sheet was discarded.
KDA_4_60111C - --There is a buildup of debris on the base of the cooler storing the bags of fries. There is dried food spill on the base of the milk cooler in the drink preparation area.
KDA_4_90311C - --By the bread toaster there is a stack of round styrofoam to-go containers that are outside of their original protective packaging, neither covered nor inverted.
KDA_6_20211A - --The lights in the walk-in-freezer are not shielded/covered.
KDA_6_50112A - --The floor below the cooking equipment has a buildup of grease and food debris on it.
KDA_7_20111B - --On the chemical shelf there is a roll of paper towels on the bottom shelf below cleaners. The assistant manager said these paper towels were used in the paper towel dispenser at the hand sink in the kitchen. COS - she moved the paper towels.

KDA_3_30212 - Food storage containers (Common Name) Except for containers holding FOOD that can be readily and unmistakably recognized such as dry pasta, working containers holding FOOD or FOOD ingredients that are removed from their original packages for use in the FOOD ESTABLISHMENT, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be identified with the common name of the FOOD.
KDA_3_30414B1 - Wiping cloths (Storage) Cloths in-use for wiping counters and other EQUIPMENT surfaces shall be held between uses in a chemical sanitizer solution at an appropriate concentration.
