  • 4-302.14 - --REPEAT: The owner has not yet purchased new chlorine test strips. She has searched for the missing ones with no luck. She said she has not yet been out of town to buy new strips.
  • 4-102.11(B)(1) - --There are two plastic grocery bags that are being used to store green peppers. The peppers are in direct contact with the grocery bags. Let the owner know that these bags are not approved for direct contact with food. She said she will get the peppers out of these bags.
  • KDA_3_30511A1 - --REPEAT: There are boxes of bagged soda pop sitting directly on the floor in front of the microwave in the kitchen.
  • KDA_3_30512 - --REPEAT: There is a storage room by the soda pop fountain. Inside of this storage room there is a toilet. The rack of boxes of bagged soda pop for the soda fountain are stored in this room. Note - the owner said that this is never used as a restroom, there is another restroom.
  • KDA_3_50116A2 - --REPEAT: The cheese in the cooler below the make table is measuring 44.1 F. The owner said it's been in here a few days and she has not had it out yet today. Corrected on Site (COS) - the cheese was discarded.
  • KDA_4_30111 - --REPEAT: The ambient temperature inside of the make table after being left closed for 30 minutes is 46.8 F. Since the previous inspection, the owner has put her old seal back on the make table and sent the other back to the company to be replaced. She will check to see if the make table is turned down far enough. For the time being, all PHF items are being moved to another cooler.
  • KDA_4_60111A - --REPEAT: The cheese container in the make table cooler has stick residue on the outside of the container. This was identified as remaining from the original purchasing of the container and was not completely cleaned off when the container was washed. The owner said she had told her staff to remove this and they had said they would do it. She will make sure this gets taken care of. There are four containers below the 3 vat sink that were originally labeled for jalapenos/banana peppers. The original labeling stickers are still on the outside of the containers. The containers are being stored as clean.
  • KDA_4_60111C - --REPEAT: The fan near the microwave in the kitchen has an accumulation of dust on it.
  • KDA_4_90311C - --REPEAT: There is a single stack of single-use styrofoam to-go containers on top of the microwave. These are outside of their original protective packaging and are neither covered nor inverted for protection
  • KDA_4_90312A - --REPEAT: There is a storage room by the soda pop fountain. Inside of this storage room there is a toilet. There are boxes of single-use cups and paper towels stored in this room. Note - the owner said that this is never used as a restroom, there is another restroom.
  • KDA_6_20215A3 - --REPEAT: The back door in the kitchen has a narrow 1/4 to 1/2 inch gap along the side that goes about halfway up the back door.
  • KDA_6_50112A - --REPEAT: There is a buildup of dust on the wall behind the pizza oven.
  • KDA_7_20111B - --There is a bottle of rubbing alcohol sitting by the microwave on the counter. The cabinets below this have clean utensils, parmesan cheese packets, and single-use cups. COS - the owner moved the rubbing alcohol upon request.

  • 4-302.14 - --The owner is unable to provide chlorine test strips today. She said she had them because the previous inspector required them, but she is unsure where they are at today. Reminding her of the 1-2-3 method for measuring bleach (1 cap to 3 gallons of water).
  • KDA_3_30511A1 - --In the kitchen there are boxes of bagged soda pop being stored directly on the floor next to the 3 vat sink.
  • KDA_3_30512 - --There is a storage room by the soda pop fountain. Inside of this storage room there is a toilet. The rack with the boxes of bagged soda pop hooked up to the soda fountain are stored in this room. Note - the owner said that this is never used as a restroom. There is another restroom that is used.
  • KDA_3_50116A2 - --There are food items inside the make table that are not within the required cold holding temperatures. They are as follows: Beef - 54.3 F Sausage - 54.3 F Cheese - 54.3 F The items were all tested individually and when the temperature problem was found, the owner said she had recently made two pizzas, but she had only had the make table open for a max of 5-10 minutes, it had been left closed the rest of the morning. Left the make table closed for 35 minutes to see if the temperature went lower and the ambient air measuring 57.6 F after the make table was left completely closed for 35 minutes. COS - the food items were discarded.
  • KDA_4_30111 - --The make table is measuring 57.6 F ambient temperature. The owner said this make table has only been opened for 5-10 minutes today. It was left closed for a full 35 minutes to see if the temperature started to come down and there was no improvement. Note - the owner said that the door seal on this make table was just replaced and wondered if this might be the problem because it does not seem to be right. She will have this checked.
  • KDA_4_60111A - --The container of cheese in the make table has sticker residue on the outside of the container that was identified as being left over from the original purchasing and left over from being cleaned since it was purchased. Corrected on Site (COS) - the owner put this container with ware washing. There are several containers that were previously used and labeled for banana peppers that are being reused to store other food items. These containers have stickers on the outside from the original labeling for banana peppers. The stickers were not cleaned off when the containers were cleaned. These containers are being used to store crushed red pepper, parmesan, sugar, ketchup, mustard, and creamer. There are also several of these containers being stored as clean on the shelf below the hand sink that still have stickers on them.

  • KDA_7_30111A - --On the retail floor there is windshield wash fluid sitting on a shelf directly next to single-use drinking cup lids. COS - the owner moved the lids. There are two bottles of Windshield washer fluid being stored directly next to boxes of soda pop cans on the retail shelf. COS - the owner moved the windshield washer fluid. On the retail shelf there are bottles of Comet stored directly next to rolls of wax paper. Made recommendations on how to rearrange this to the owner.
  • KDA_7_30111B - --On the retail shelf there is Armor AU Protectant Wipes and glue stored directly above boxes of ziploc bags. Let the owner know this needed to be moved and made recommendations for rearranging.
  • KDA_Defacto - --Hot Case: Chicken Taquito - 137.9 F --Milk delivery - 40.4 F --Owner has obtained a variance for the lack of a mop sink. --Ownership verified. --Single-use plastic banana pepper containers are being reused to store other food items. Note - this is not recommended as these containers are not as sturdily built and can break down faster. --The bleach below the sink that is being used to sanitize dishes does not have label instructions for sanitation. The bleach bottle lists that a rinse is required after use, prior to air drying. Letting the owner know that she needs to start using bleach that is labeled for sanitation on food contact surfaces. The bleach hanging above the 3 vat sink (dirty dishes vat) may never be above the rinse water or the sanitizer water vats. Also, this bleach is measuring > 200ppm and should be diluted prior to being used on food contact surfaces. --The owner puts left over hot items in the cooler in the kitchen. --The ready-to-eat beef and sausage pizza toppings in the make table were pulled from the freezer yesterday, per the owner. --Windowed Reach-in/Walk-in-Cooler: Beef, Cheese, & Bean Burrito - 36.4 F White cooler (Kitchen): Pizza - 42.7 F Freezers are good. There are 3 coolers that do not have PHF items inside.
