- KDA_3_30212 - --There are two unlabeled shakers by the stove that were identified as salt and pepper.
- KDA_3_30611 - --During serving, the food is set out on a steam table in the dining area. The food is served uncovered and unprotected from the consumers coming through the line to be served. Letting staff know that they either need to have a sneeze guard or provide another form of protection for the food being served.
- KDA_4_20211A2 - --There are two cracked plastic food containers on a shelf in the back of the kitchen. The lid to the pepper container is cracked across the top. The lid to the caramel soy container is cracking in the corners along the handle and is duct taped where these cracks are.
- KDA_4_50111A - --The "True" cooler in the kitchen is dripping from the condenser. The drip is being caught in containers so that it does not contaminate any food products in the cooler.
- KDA_4_60111A - --There is a salad dressing bottle being reused to store salt in the dry storage area. The sticker from when this bottle was labeled for salad dressing is still on the bottle after the bottle was cleaned to be used for salt. Letting the manager know that this sticker should have been removed during the cleaning process and needs to come off.
- KDA_4_70211 - --The sanitizer in the 3 vat sink is showing 0ppm chlorine when tested. An employee is doing dishes and using this for the sanitation of the dishes. Corrected on Site (COS) - the sanitizer was remade to 50ppm and the dishes re-sanitized.
- KDA_5_50117 - --The unisex handicapped restroom does not have a covered trashcan available.
- KDA_6_20215A3 - --There is a 1/8 to 1/4 inch gap between the two front doors where sunlight can be seen shining through while they are closed.
- KDA_7_20111B - --There is a bottle of dawn dish soap stored in a container on a rolling cart. There are clean dishes stored on this rolling cart on shelves directly below the dish soap. Letting the manager know this needs to be stored elsewhere.
- KDA_Defacto - --Chlorine test strips were available in the kitchen. --Cooking Chicken - 191.8 F --Freezers are good. Whirlpool Cooler: No PHF items. True Cooler: Egg - 37.9 F Black Cooler (Dining): No PHF items. --Frozen meats are being thawed in the cooler. --Please note - the single use containers such as the seasoning containers, coffee grounds containers, and salad dressing containers that are being reused to store other items throughout the kitchen are not recommended for reuse. These containers are not as durable and may break down more quickly. --The bleach for use on the dishes in the kitchen specifically says on the label, "Do not use on silverware." Recommending they get bleach that is approved for use on all food contact surfaces. --The wet wiping cloth is hanging over the edge of the bucket of sanitizer, rather than being stored inside of the sanitizer. Also - the sanitizer in the bucket is showing 0ppm chlorine.

- KDA_3_30611 - FOOD protected - Except for nuts IN the Shell and Whole, Raw fruits and vegetables that are intended for hulling, peeling, or washing by the CONSUMER Before consumption, FOOD on display shall be protected from contamination by the use of packaging- counter, service line, or salad bar FOOD guards- display cases- or other effective means.
