COS-discarded the bag of hash browns voluntarily. KDA_3_30212 - --On the counter top there was a squirt bottle with clear liquid. The cook said it was water for the grill. COS-labeled bottle water. There was also a clear plastic shaker with a white powder substance. The cook said it was powdered sugar. COS-labeled the shaker. KDA_3_50116A2 - --In the General Electric refrigerator (to the right of the 4 vat sink) there was bags of raw chicken at 73.1F, 70.8F and 46.4F. The cook said they were thawed under running cold water in the 4 vat sink from 5:00am just prior to placing them in the refrigerator before 11:00am (past 4 hours). COS-discarded. The ambient air temperature of the unit was reading 41.5F. KDA_3_50118A2 - --In the Whirlpool refrigerator there was a plastic food grade container of cooked ready to eat roast beef without a date. The cook said it was made on 10/11/15. COS-date marked the item. In the General Electric refrigerator (to the right of the 4 vat sink), there was an open package of turkey without a date. The cook said it was opened on 10/9/15. There was also an open bag of cut salad without a date. The cook said it was opened on 10/11/15. COS-date marked both items. KDA_4_20211A2 - --In a utensil holder there was one rubber spatula that had cracked/chipped edges. COS-trimmed the edges smooth. KDA_4_60111C - --The General Electric stand up freezer has soiled residue on the side where it is opened. There is dust build up on the inside of the hood above the grill. There is grease build up on the sides of the fryer. KDA_4_60211E4 - --There was minor mold build up in the pop nozzles at the soda fountain machine. There was minor dried food debris inside of the plastic dispensers of the Cappuccino machine. KDA_4_90311A - --In the storage room there was one box of foam cups that were being stored directly on the floor. KDA_4_90311B - --On a shelf above the large mixer, there were ceramic bowls and plates that were facing up and not protected. COS-placed a clean cloth to cover them. KDA_5_20515B - --The faucet at the 4 vat sink leaks when water is supplied. KDA_6_20211A - --In the dry food storage room the light fixture does not have a protective cover. In the waitress area there are 6 ceiling light covers missing and the light fixtures are not protected. KDA_6_20215A3 - --There is a 1/2" gap along the bottom of the back exit door. This could allow the entrance for pests. KDA_6_50111 - --The ceiling is exposed in the storage room but there is plastic stapled to wood beams to keep the insulation from being exposed. There are 6 light covers missing in the waitress area. There are 6 ceiling tiles that are water stained in the dining area. KDA_6_50112A - --There is dried rodent excreta next to the carbonator pump for the soda system in the waitress area. There is grease build up on the floor below the fryer. KDA_7_10211 - --On a metal table there was a white plastic bucket with clear liquid and not labeled. The cook said it was sanitizer water. COS-labeled bucket.
