At the ice machine, a single use food grade bucket being re-used for ice had grime on the inside top and outside. There was also a sticker on the outside of the bucket (removed to warewashing area). On a shelf stored as clean, four stainless steel food pans had food on them, and a stainless steel food container stored as clean had debris on the inside (removed to 3-vat sink). The table mounted can opener blade had dried food on it (removed to 3-vat sink). Also in the produce department, dried food was on an onion slicer stored as clean (asked deli manager to clean). KDA_4_60111C - --In the produce walk in cooler, the meat department walk in cooler, and the meat department cutting area, dust is built up on the cooling unit fan blade guards. KDA_4_60211E4 - --(b) - a small amount of mold is in the ice machine bin area, on an interior surface of the door (located above the ice). KDA_4_70211 - --The produce manager, when asked, said a cutting board was sprayed with 409 cleaner, then rinsed, (no sanitizing step). Reviewed proper procedure with the manager and had him take the board to the deli to wash-rinse-sanitize.Also, the 409 cleaner is not an approved chemical to use on utensils. As per a manager, they have a hired pest control service that sets out bait boxes and traps. Pest control methods observed were rodent bait boxes and mechanical traps. KDA_6_50112A - --At the deli, there is a black substance built up in a cabinet under a handsink and debris in cabinets at the deli service counter cabinets. Dust and debris are in several of the deli cabinets. There is a small open pit on the floor by the deli case that has food debris in it. Grime and soil are on the floor behind the ice machine. A large amount of ice is built on the deli walk in freezer floor. Food debris in on the floor in the deli walk in cooler. In the deli, grease is built up on the floor, wall, and electrical boxes by the fryer. Debris is on the floor in the bakery behind the large ovens. In dry storage, debris is on the floor under shelving in numerous locations. Not cleaning up rodent droppings (fresh and old), old droppings are in numerous locations on the floor and shelving in dry storage. KDA_7_20111B - --In the deli, a jug of quaternary ammonia sanitizer is mounted directly above the sanitize vat. It cannot be mounted above the sanitize vat or the rinse vat as per the KDA office. In the produce per area, a detergent and sanitizer dispensing unit is mounted above the sink, this sink is used for produce washing, this unit must be removed. KDA_7_30111A - --On a retail shelf, charcoal starter fluid stored next to and touching packages of single use plastic cups. KDA_7_30111B - --On a retail shelf, coffee pot cleaner stored above coffee filters and creamers.
