KDA_3_10111 - --Located in the walk in cooler (WIC), there were two opened gallons of milk that were not dated. The kitchen employee said that she did not know when they were opened. Corrected on site (COS)- discarded. KDA_3_30211A1b - --Located in the WIC, there was a container of raw beef stored on a shelf over shelves with bottled Gatorade, bottled Frappuccinos, and bottled water. COS- moved to bottom shelf. KDA_3_30212 - --Located on the shelf above the MT1, there was an unlabeled container of white powder and an unlabeled shaker container of white powder which the kitchen employee identified as salt and powdered sugar, respectively. Located in the MT2, there was an unlabeled bottle of orange liquid, an unlabeled bottle of brown liquid, and an unlabeled bottle of yellow liquid which the kitchen employee identified as hot sauce, bbq sauce, and butter, respectively. Located on the table with the grill, there was an unlabeled bottle of clear liquid which the kitchen employee identified as water. Located on the storage shelf next to the WIC, there were two unlabeled containers of white powder and an unlabeled container of brown powder which the kitchen employee identified as sugar, flour, and cinnamon sugar, respectively. Located in the walk in freezer (WIF), there was a box of funnel cakes, a box of pretzels, and a box of French fries stored on the floor. Located in the storage room, there was a box of Sierra Mist soda syrup stored on the floor. KDA_3_50118A2 - --Located in the WIC, there was an opened container of chocolate milk with no date. The kitchen employee said that it had been opened on 12-13-15. COS- dated. KDA_4_10111A - --Located in the Turbo Air reach in freezer, there were non food grade containers used to store chicken patties, grilled chicken bites, pretzel bites, chicken nuggets, chicken tenders, mini corn dogs, and mozzarella sticks. These foods were in direct contact with the non food grade containers. COS- wrapped foods in a food grade plastic wrap before putting back into the containers. Educated employee on using food safe containers or bags. KDA_4_20211A2 - --Located on the dish rack, there was one plastic food container that was cracked. COS- discarded. KDA_4_60111C - --There is grease and food debris on the front of the pizza oven. There is an accumulation of grease on the front, sides, and inside the doors of the fryers. The fan guards of the WIC and WIF are soiled with dust. KDA_4_60211E4 - --There is an accumulation of mold on the inside rim of the ice machine located in the storage room.
