KDA_3_10111 - --In the small RIC below the front counter there was an open 1/2 gallon of milk without a known date. COS-discarded.
KDA_3_50116A1 - --On the buffet line there was a metal sheet pan of pepperoni pizza at 124.7F and a metal sheet pan of a calzone at 118.6F. They were tempted around 12:10pm and employee said they were placed 30 min. prior. COS-voluntarily discarded and made new product along with adjusting the temperature dial to the warmer.
KDA_4_50111B - --The left and middle door to the reach in cooler (below make table) gaskets are torn. The door gasket towards the bottom of the walk in cooler is torn. This does not provide a good seal.
KDA_4_60111A - --On the utensil rack above the 3 compartment sink there was a large produce slicer with dried food debris on the blades and stored clean. The large can opener connected to the metal prep table also had dried food debris on the blade and was not used to today. COS-moved both items to the ware wash area.
KDA_4_60111C - --There is mold on the door seal to the walk in cooler.
KDA_6_20211A - --There are 2 light fixtures in the back food/utensil storage room that do not have a protective cover.
KDA_6_20215A3 - --The back exit door located in the storage room has a 1/2" gap along the top and right side. This could allow the entrance for pests.
KDA_6_30112 - --There were no disposable paper towels at the handsink located at the front. COS-an employee replaced a roll in the dispenser.
KDA_6_50112A - --There are dead insects on the floor in the back storage room near the hot water tank and the electrical boxes.
