KDA_3_50114A2 - --Gravy in RIC was at 44F. PIC stated the gravy had been prepared the previous evening and had been place in RIC. It had been cooling for longer than 6 hours. COS-discarded. KDA_3_50116A1 - --In the hot case, breakfast burritos were at 108F, 103F. Egg rolls were at 132F. PIC stated they had been in unit for approximately two hours. COS-discarded. KDA_3_50118A2 - --Gravy in the RIC was not dated. PIC stated that it had been prepared yesterday morning, more than 24 hours earlier. COS: discarded. KDA_4_20211A2 - --A food container was cracked/broken. COS: discarded. KDA_4_204112E - --Thermometer in hot case was scaled in 25 degree increments. KDA_4_30111 - --Kitchen RIC had an ambient temperature of 54F. Temperature of gravy stored in unit was greater than 41F. KDA_4_30212A - --Kitchen RIC was not equipped with a thermometer. KDA_6_501111A - --There are fresh rodent droppings too numerous to count (approximately 200 - 250) throughout the upstairs storage area. There are approximately 10-15 fresh rodent droppings in the storage room under stairs. There were three fresh rodent droppings in the storage closet with the bag-in-boxes. KDA_6_501111B4 - --There are boxes of assorted mechanical, and other supplies stored on the stairs leading to the upstairs storage area. KDA_6_50112A - --Floor under warewash sink is soiled with grease and food debris. Ceiling vents and surrounding tiles in kitchen food prep area are soiled with an accumulation of dust and dirt. There is fungus growing on the wall in the bag-in-box storage area and in the cupboard under the Pepsi soda fountain machine.
