KDA_7_20111A - --A spray bottle of cleaner was stored next to single-serve food paper on storage shelf under a prep table. KDA_7_20612 - --Open face rodent bait trays, and rodent bait packets were in the basement storage area. The rodent bait packet, and opened rodent bait tray were not in to bait stations. COS, PIC removed all the rodent bait. KDA_Defacto - --Cooked tamales 63F were stored on a prep table. KDA_2_40111 - --An opened no lid drinking cup of coffee was on the prep table. COS, drinking cup removed. KDA_3_10111 - --Completely thawed fish in ROP without an opening in the package to allow oxygen were in the front display cooler. PIC stated the fish had been in the cooler since the evening of 12/17/15. COS, the fish removed from the ROP. KDA_3_30211A1b - --Raw shrimp in an opened face container was stored on top of a container of butter in the reach in cooler. COS, raw shrimp removed. KDA_3_30412B - --Bowls without handles were stored bins of beans, salt and sugar. KDA_3_30511A1 - --Bags of rice, boxes of tortillas, and herbs were on the floor. KDA_3_50116A2 - --Raw beef 45.3F, mozzarella cheese 48F, and muenster cheese 48F were in the front display cooler (ambient 43F). COS, items discarded. KDA_4_60111A - --Dried food debris was on the blades of knives that were stored as clean on the magnetic wall strip. Dried food debris was inside of meat grinder. KDA_6_501111A - --12 fresh rat droppings were between reach in freezer. 18 fresh rat droppings were in a storage room on the floor next to the furnace. 6 Fresh mouse droppings were on the behind the vegetable storage shelf of the retail store. PIC provided pest control invoice with a service date of 12-4-15. KDA_6_501111B4 - --The basement storage area full of old boxes, and old equipment. KDA_6_50112A - --Dried food debris under food storage racks, under make table, around the stove area, under the retail food storage racks in the retail store area. KDA_7_20111B - --Raid chemical treated fly paper was stored above the meat grinder. COS, chemical removed. KDA_7_20612 - --8 open face rodent bait trays were throughout the basement storage area into tamper-resistant bait stations. KDA_7_30111B - --Bottles of soda were stored below a hand soap.
