  • KDA_3_50114A2 - --In the reach in cooler below the make table there was a metal bowl with beef tamales at 46.3F, 47.4F and 48.2F and made on 12/02/15. There was also a plastic food grade container of chili relles at 43.8F and made on 12/02/15. There was a metal food grade container of cooked carnitas at 49.1F and made on 12/02/15. They were not able to cool down to 41F within 6 hours. COS-discarded all of the items. The ambient air temperature of the unit was reading 43.5F towards the top and 46.0F towards the bottom of the unit. The cook adjusted the temperature gauge and now the unit is reading 37.0F.
  • KDA_3_50116A2 - --In the make table there was a metal food grade container of shredded lettuce at 47.2F and a metal food grade container of shredded cheese at 44.1F. The lid to the make table was left open during lunch rush. In the reach in cooler below the make table there was a plastic container of raw pork chorizo at 53.9F and a plastic food grade container of taco beef at 49.5F. The door to the reach in cooler was not shutting properly because it was getting caught on the edge of a metal table and the door was opened and closed due to the lunch rush. The ambient air temperature of the unit was reading 43.5F towards the top and 46.0F towards the bottom of the unit. COS-adjusted the temperature gauge and now the unit is reading 37.0F (Note: the food has been in the unit less than 4 hours). In the Haier refrigerator there was an open gallon of milk at 45.9F and 4 tubs of sour cream at 44.4F, 47.2F, 46.1F and 46.9F. The temperature gauge in the unit was adjusted when the food was put away yesterday (in the unit longer than 4 hours). COS-discarded all of the PHF in the unit. The temperature gauge was adjusted and the ambient air temperature of the unit is reading 39.9F. In the cooler next to the grill there was a plastic container with individually wrapped raw chicken at 48.8F and a plastic container with individually wrapped raw beef at 48.7F. Both products were above the ice bath and in the cooler less than and hour. COS-removed excess product into another reach in cooler.
  • KDA_3_50118A1 - --In the True RIC (Pepsi-in the kitchen) there was a container of tomato puree from a can and date marked 11/02. The person in charge said that was not the correct date and that it was opened on 12/02. COS-date marked the item correctly. (Note: this was the only item not date marked correctly).

  • KDA_3_10111 - --In the Randell RIC there was a package of corn tortilla shells with mold growing on them. COS-discarded.
  • KDA_3_30211A1a - --In the reach in cooler below the make table there was a ceramic bowl with 3 raw hard shell eggs being stored directly above a bag of ready to eat shredded lettuce. COS-removed the eggs. (Note: no leakage detected).
  • KDA_3_30211A1b - --In the Randell RIC there was a box of raw hard shell eggs being stored on top of a box of ready to eat tortilla shells. COS-rearranged items. (Note: no leakage detected).
  • KDA_3_30511A1 - --Down in the basement there was a bag of salt being stored directly on the floor. COS-placed on a shelf.
  • KDA_3_50116A2 - --In the True RIC at the waitress station there was a plastic pitcher of salsa (made on site) at 44.6F. It was pulled from the True reach in cooler located in the back near the ware wash area around 11:30am and was tempted around 2:45pm (less than 4 hours). COS-placed in a different reach in cooler. The ambient air temperature of the unit was reading 36.9F after the person in charge adjusted the temperature gauge.
  • KDA_3_50118A1 - --In the True reach in cooler there was a large plastic container of ground beef date marked 11/10/15 (held past 7 days). An employee said that was not the correct date and that it was made on 11/22/15. COS-date marked the item correctly.
  • KDA_3_50118A2 - --In the True reach in cooler there was a plastic container of canned jalapenos, carrots and onions without a date. An employee said it was opened on 11/22/15 around 11:00am (past 24 hours). COS-date marked the item.
  • KDA_3_60311A - --There was no Consumer Advisory (Disclosure and Reminder) available on the menus or poster at the time of the inspection. COS-printed a consumer advisory poster and asterisked the items that can be "cooked to order".
  • KDA_4_10111A - --In the kitchen on a shelf there was a plastic Sterilite non-food grade container being used to store tortilla chips. COS-placed chips in a food grade container. In the back room there was a large blue plastic Sterilite non-food grade container being used to store tortilla chips. COS-placed chips in a food grade containers.
  • KDA_4_60111A - --The large can opener connected to the table had dried food debris built up on the blade and was used yesterday (holds bacteria). COS-moved to the ware wash area. On a shelf below a metal table there was a plastic rubbermaid container with a sticker left on the side and stored clean (can hold bacteria). On the utensil rack there were three (1/3) metal food grade pans with dried food debris on the inside and outside and also had stickers left on the containers. There was one square metal container with a sticker left on the outside and stored clean. There was one large metal pot with a sticker left on the outside and stored clean. COS-moved all of the items to the ware wash area.
  • KDA_4_60211E4 - --There is minor mold build up on the Diet Dr. Pepper pop nozzle. (Note: cleaned daily).
  • KDA_4_90311A - --Down in the basement there was one box of single use to-go containers being stored directly on the floor. COS-placed on a shelf.
  • KDA_4_90311B - --On the utensil rack there were three (1/3) metal food pans being stacked clean/wet and not able to properly air dry. COS-moved to the ware wash area.
  • KDA_5_50117 - --In the unisex restroom there was no covered receptacle for sanitary napkins. COS-placed a covered waste basket in there.
  • KDA_6_20214 - --The unisex restroom door is not self-closing. An employee had it disconnected due to it being too difficult for elders to open the door. (Note: talked about hooking a spring to help retract the door).
  • KDA_6_30111 - --There was no soap out & available at the handsink located in the waitress station. COS-placed soap at the handsink.
  • KDA_7_10211 - --Below the 3 compartment sink, there was a white plastic bucket with clear liquid and not labeled. An employee said it was degreaser. COS-labeled the bucket.
