5-202.14 - --There was a green hose attached directly to the water line above the mop sink without any backflow prevention attached. The end sprayer of the hose is laying inside the basin of the mop sink below the flood rim. Discussed necessary backflow prevention with PIC, including education on continuous pressure if the sprayer will still be attached.
KDA_3_50118A2 - --There was an opened gallon of milk in the front reach in cooler without any date marking. PIC stated the milk was opened yesterday 10/12 at approximately 10AM. COS- employee correctly date marked. There was a container of precooked chicken breasts and two containers of hot dogs in the reach in cooler of the make table without any date marking. Employee stated the chicken was prepared yesterday 10/12 at approximately 10AM and the hot dog packages were opened Sunday 10/11. COS- employee correctly date marked. There were two opened packages of sliced turkey in the walk in cooler without any date marking. Employee stated the turkey was opened yesterday 10/12 at approximately 10AM. COS- employee correctly date marked.
KDA_4_60111C - --There is dust build up on the fan guards in the walk in cooler. There is dust build up on the vents of the soft serve units at the front counter.
KDA_4_90311A - --There were several single service trays stored directly on the ground in the back storage area.
KDA_5_50117 - --There is no covered trash receptacle in the women's public restroom.
KDA_6_50116 - --There was a mop with a handle stored in the mop bucket not allowed to properly air dry.
KDA_7_20111B - --There was a container of QT sanitizer for wiping cloths on the back storage shelf directly above several boxes of single service gloves. COS- PIC rearranged the shelves so that the chemicals were on a lower shelf.

Per the manufacturer label the proper cleaning concentration should be between 200 and 400ppm. Employee remixed, no dishes were being cleaned at time of inspection. QT test strips were provided. --There is a pest control company contracted, no paperwork provided. No activity. --Wiping cloth QT measured at 300ppm, test strips provided.
