  • KDA_3_30211A1b - --In the WIC, meatloaf (raw ground beef) was stored on a rack above RTE cooked pasta. COS PIC removed pasta from rack and placed on pasta shelf.
  • KDA_3_30212 - --In the kitchen prep area, rice, brown sugar, seasoned flour, and flour did not have a common name label.
  • KDA_3_40113 - --In the parmesan station, fried egg plant had a measured temperature of 119F. The pan was taken out of the oven. COS PIC was educated. Pan of egg plant was put in oven for rapid temperature increase. At the end of inspection, egg plant had a measured temperature of 168F.
  • KDA_3_50118A2 - --In the WIC, 5 containers of 2% milk did not have a open or discard date. PIC said they were opened 12/18. COS Milk cartons were date marked. In the dessert area, a carton of 2% milk did not have a open or discard date. Employee said the milk was opened 12/20 in the morning. COS Milk was date marked. In the RIC, a metal pan of crepe batter did not have a prep date or time. PIC said it was prepared in the morning around 9am. COS PIC discarded the batter.
  • KDA_3_50119B2 - --In the dessert area, butter was had an measured temperature of 78F. PIC said the butter had been sitting out since 10am. The butter was not marked with time. COS PIC marked butter with time.
  • KDA_4_20211A2 - --Stacked and stored as clean, a large white food contact container had a crack on the rim of the container. COS Container was discarded. Stored as clean, pitchers had cracks on the interior surface. COS Pitchers was discarded.
  • KDA_4_60111A - --Stacked and stored as clean, food contact metal pans and skillets had visible dried food on the interior surface. The metal pans also had old date marking stickers on the exterior surface. Stacked and stored as clean, a large white food contact container was stored on a storage rack with old date marking stickers. COS Container was removed to be washed, rinsed, and sanitized. Stored as clean, pitchers had old date marking stickers on the exterior surface. COS Pitchers was removed to be washed, rinsed, and sanitized. Stacked and stored as clean on the bakers rack, was food contact containers that had visible dried food debris and old date marking stickers. COS Containers was removed to be washed, rinsed, and sanitized. The can opener had visible dried food on the blade. COS Can opener was removed to be washed, rinsed, and sanitized. Stored as clean, a food contact chopper/blender was stored on a storage rack had visible dried food debris on the interior and exterior surface.
  • KDA_4_60211C - --A slicer was stored as clean on a counter had visible dried food on the back of the blade and the front of the slicer. PIC was not sure if the slicer was used earlier in the morning. COS Slicer was cleaned during inspection.
  • KDA_4_90311B - --Stacked and stored a clean, metal pans were stacked wet after being washed, rinsed, and sanitized.
  • KDA_5_20315 - --2 carbonator fountain machine did not have a vented dual check valve, back flow prevention device at the wait staff area.
  • KDA_5_20515B - --In the bar area, a pipe underneath the handwashing sink leaks when water is turn on.
  • KDA_5_50117 - --The women's restroom did not have a covered receptacle for sanitary napkins.
  • KDA_6_30112 - --The handwashing sink near the warewashing area did not have paper towels. COS PIC provided paper towels during inspection.
  • KDA_6_50112A - --In the back kitchen area, the bar and throughout the varies station, there was standing water on the floor as if the floor is not properly sloped for drainage. In the back kitchen area, there were broken tile by the stove that had standing water inside the crack.
